The Short-term Effect Observation Of Clinical Application Of Rotating Hinge Prosthesis

Objective: To discuss short-term effect observation of clinicalapplication of rotating hinge prosthesis.Methods: From July 2002 to April 2005, Seventeen knees in 17patients with severe deformity and instability received rotating hinge kneeprosthesis for total knee arthroplasty. There were 8 males and 9 females.The left joints in 10, right in 7, the age of the group were from 41 to79 years old,with the average of 59 years. All patients were admitted dueto pain, the course of disease were form 1 to 7 years. There wereosteoarthritis 10 cases, rheumatoid arthritis 5 cases, traumatic arthritisafter post-operation of left femur fracture 1 cases, traumatic arthritis afterpost-operation of left fracture of tibial plateau with the injury of anteriorcruciate ligament, meniscus medialis and medial collateral ligament.According to HSS scoring system, the preoperative score was from 36 to58, with the average of 48.6. The preoperative flexed motion range ofarticulation were from 21°to 80°,with the average of 57.4°.Results: All patients were follow-up from 7 months to 3 years,with the average of 23.6 months. There were no thrombogenesis of veinsof lower extremity, no pulmonary embolism, no palsy of peroneal nerve,no fracture, no breakage of extended knee structure and othercomplications. 1 patient was infected at 3 months postoperatively, theprosthesis was dislodged, antibiotic-impregnated cement was filled inknee joint, two-stage exchange arthroplasty was expected. At the last follow up, the HSS score was from 78 to 98 in 16 patients, with theaverage of 91.1. The flexed motion range of articulation were from 75°to100°at 2 weeks postoperatively, with the average of 85.2°in 16 patients.The flexed motion range of articulation were from 85°to 123°at the lastfollow-up, with the average of 108.3°in 16 patients. The difference ofHSS score and motion range of articulation between pre-operative and thelast follow-up were significant (P＜0.05).Conclusion: The short-term outcome of rotating hinge kneeprosthesis was good, a long term follow-up was necessary. The assuranceof curative effect including stict sugery indication, skilled operation andcorrect treatment after operation.
