OBJECTIVE:The study intended to evaluate the effect of the project' practice and to provide the theoretical foundation for intervention in Countryside & Village Medical Institutions of SHANXI Province which are project areas of HealthⅧproject by surveying the quality of prescription;The aim was to find out the problem in prescribing depending on investigating the physicians' prescribing behavior.METHODS:2760 Outpatient prescriptions were random chosen from Countryside & Village Medical Institutions which are project areas of HealthⅧProject Counties in SHANXI Province during the period of January to June in 2001 and in 2005,and the information was collected on a specifically designed form.The calculation of indicators referred to the methods and standard in《How to Investigate Drug Use in Health Facilities-Selected Drug Use Indicators》,which was designed by WHO/INRUD.Statistics analysis carried out Excel 2003 and SPSS 11.5 statistical package and statistical methods used t test and x~2 test et al.RESULTS:1,the analysis of prescribing on quality:Percentage of prescription qualification was 18.5%and 0.3%in 2005 and in 2001,respectively;Among the prescription head part,text and tail part qualified were 52.4%,26.5%,31.6%in 2005,19.8%,14.7%,6.0%in 2001,respectively,there was significant difference between two groups(P<0.01). 2,the analysis of prescription indicator:The average number of drugs per encounter in 2005 and in 2001 was 3.0 and 3.8 respectively,there was significant difference between two groups (t=8.699,P<0.001);86.1%and 76.4%of drugs were prescribed by generic name in 2005 and in 2001 respectively,there was significant difference between two groups(x~2=128.159, P<0.001);Percentage of encounters with an antibiotic prescribed in 2005 and 2001 was 47.9%and 67.7%respectively,there was significant difference between two groups (x~2=100.223,P<0.001);Percentage of encounters with an injection prescribed in 2005 and in 2001 was 37.4%and 54.8%respectively,there was significant difference between two groups(x~2=75.657,P<0.001);Percentage of hormone prescribed in 2005 and in 2001 was 13.2%and 20.1%respectively,there was significant difference between two groups (x~2=21.715,P<0.001);Percentage of drugs prescribed from essential drugs list or formulary in 2005 and in 2001 was 86.0%and 80.0%respectively,there was significant difference between two groups(x~2=53.181,P<0.001);The average drug cost per encounter in 2005 was 29.9 yuan,of which antibiotics and injections comprised 63.2%and 89.8%, respectively;The average drug cost per encounter in 2001 was 21.4 yuan,of which antibiotics and injections comprised 52.2%and 89.6%,respectively,there was significant difference between two groups(t=-6.198,P<0.001);Percentage of encounters with infusion prescribed was 32.0%and 48.2%in 2005 and in 2001,respectively,there was significant difference between two groups(x~2=68.215,P<0.001).CONCLUSION:The quality of prescribing and the level of the rational drug use were higher after practicing project in project areas of HealthⅧProject Counties in SHANXI Province.The practice of project got effect but irrational drug use was still serious.Need further to strengthen the intervention for enhancing rational drug use's level.
