| Objective: The gastric carcinoma is one of the most common malignant tumors in our country. The rate of incidence and death is the highest in all kinds of the digestive system tumors. Disequilibrium of multiplication and apoptosis plays an important role in development of gastric cancer. Peroxisome proliferaterator-activated receptors are very important as nuclear transcription factors for cell differentiation and apoptosis. However, a lot of patients died of the invasion and metastasis of tumor cells. MMP-7 is one of important members in matrix metaloproteinase family. It can degrade the extracellular matrix and make tumor cells metastasis.Ligands of PPARγcan regulate down the expression of MMP-7 through some pathway. But now there are little reports about the expressions of MMP-7 and PPARγin gastric carcinoma and regional lymph node tissues. And there is no report on their correlation research. Therefore this research detects the expressions of MMP-7 and PPARγin gastric carcinoma, gastric mucosa and regional lymph node tissues by S-P immunohistochemical technique, analyzing their contribution in the clinical biological behavior of gastric carcinoma. So we can provide the theoretical foundation for prognosticating the invasion and metastasis of gastric carcinoma, guiding the reasonable comprehensive treatment.Methods: In this study, we select 73 samples of gastric carcinoma, 73 samples of first station regional lymph node and 73 samples of normal gastric mucosa. All patients had gastrectomy in the third department of surgery of the 4th hospital of Hebei Medical University from October 2005 to August 2006. All gastric carcinoma clinical records registered according to gastric carcinoma treatment criterion. The expressions of MMP-7 and PPARγin gastric carcinoma tissues, regional lymph node tissues and normal gastric mucosa were detected by S-P immunohistochemical technique in this study. The relationship between the expressions of MMP-7 and PPARγand the biological behavior of gastric carcinoma were analyzed, including patients'sex, age, tumor location, size, gross type, differentiation degree, the depth of invasion, lymph node metastasis and TNM stage etc. All data are analyzed by statistics software SPSS 12.0, and P<0.05 was considered significant.Results: 1 The positive staining of MMP-7 was located in cytoplasm, and the positive staining of PPARγwas located in nucleus. The positive rates of MMP-7 were 67.12% in tissues of gastric carcinoma, which were significantly higher than the rates of 6.85% in normal gastric mucosa (p<0.01). PPARγprotein positive expression rate in gastric carcinoma tissue was 43.84%, which was higher than 2.74%, the expression rate in normal gastric mucosa. The comparison of the two shows significant difference (P<0.01).2 The expressions of MMP-7 in gastric carcinoma tissues show no significant relevance to patients'sex, age, tumor location, gross type and differentiation degree (P>0.05). 74.51% of MMP-7 in gastric carcinoma tissue existed in the tumor bigger than 5cm, and the percentage was higher than 50.00 % in the tumor smaller than 5cm. The comparison of the two shows significant difference (P<0.05). As to the infiltration depth, the positive expression rate of T3+T4 was 73.44%, which was higher than 22.22%, the expression rate of T1+T2. The comparison of the two shows significant difference (P<0.01). The positive expression rate of tumors with lymphoid node metastasis was 88.64%, which was higher than 34.48%, the expression rate of tumors without lymphoid node metastasis. The comparison of the two shows significant difference (P<0.01). The positive expression rate in clinical stages III-IV was 86.96%, which was higher than 33.33%, the expression rate in clinical stages I-II. The comparison of the two shows significant difference (P<0.01).3 The expressions of PPARγin gastric carcinoma tissues show no significant relevance to patients'sex, age, tumor location and gross type (P>0.05). 52.94% of PPARγin gastric carcinoma tissue existed in the tumor bigger than 5cm, and the percentage was higher than 22.73 % in the tumor smaller than 5cm. The comparison of the two shows significant difference (P<0.05);As to the differentiate extent, the moderately and poorly differentiated positive expression rate was 56.41%, which was higher than 29.41%, the well and moderately differentiated rate. The comparison of the two showed significant difference (P<0.05); As to the infiltration depth, the positive expression rate of T3+T4 was 48.44%, which was higher than 11.11%, the expression rate of T1+T2. The comparison of them showed significant difference (P<0.05). The positive expression rate of tumors with lymphoid node metastasis was 59.09%, which was higher than 20.69%, the expression rate of tumors without lymphoid node metastasis. The comparison of the two showed significant difference (P<0.01); The positive expression rate in clinical stages III-IV was 63.04%, which was higher than 11.11%, the expression rate in clinical stages I-II. The comparison of them showed significant difference (P<0.01).4 In gastric regional lymph node tissues, expressions of MMP-7 or PPARγin lymph nodes without metastasis were not observed. The expression rate of MMP-7 in lymph nodes with metastasis was 90.91%. The expression rate of PPARγin lymph nodes with metastasis was 79.55%. There was no significant difference on the expression of MMP-7 in gastric regional lymph nodes with metastasis (90.91%) and in gastric carcinoma tissues(88.64%) (P>0.05). The expressions of PPARγin lymph nodes with metastasis(79.55%) are significantly higher than those in gastric carcinoma tissues (59.09%) (P<0.05). 5 The expressions of MMP-7 and PPARγhave positive correlation in gastric carcinoma tissues (rs = 0.299, P<0.05).Conclusion:1 The expressions of MMP-7and PPARγin gastric carcinoma tissues are significantly higher than those in normal gastric mucosa. It shows that the two were in administrative relation and cooperate in the process of gastric carcinoma development. To detect MMP-7and PPARγwould be useful molecular marker for gastric diagnosis.2 The expressions of MMP-7 in gastric carcinoma tissues show no difference in patients'sex, age, tumor location, gross type and differentiation degree. There was significant correlation between protein MMP-7 positive expression in gastric carcinoma tissues and the increasing of gross tumor volume, the depth of invasion to deeper, lymph node metastasis, TNM stage. This shows that they both can be the important index of judging gastric carcinoma prognosis. The expressions of PPARγin gastric carcinoma tissues show no difference in patients'sex, age, tumor location, and gross type. There was significant correlation between protein PPARγpositive expression in gastric carcinoma tissues and differentiation degree to lower, the increasing of gross tumor volume, the depth of invasion to deeper, lymph node metastasis, TNM stage. It shows that the abnormal expressions of MMP-7 and PPARγin gastric carcinoma tissues play important roles in invade and metastasis of gastric carcinoma. The expressions of MMP-7 and PPARγare correlated with clinical pathology progress of gastric carcinoma.3 In lymph node metastatic foci, the expressions of protein PPARγwere significantly higher than in their primary foci of gastric carcinoma. It's likely that there is difference between primary foci of gastric carcinoma and lymph node metastatic foci. PPARγis likely to be related factors for gastric carcinoma metastasis.4 The expressions of MMP-7 and PPARγtissues have positive correlation in gastric carcinoma tissues. It shows that there is close relationship between the expressions of MMP-7 and PPARγin gastric carcinoma. It is possible that they affect the process of gastric carcinoma development,invasion and metathesis. |
