| Object:To evaluate the clinical effect of posterolateral bone graft fusion to pretend the adjacent segment degeneration after lumbar fusion and discuss the indication.Method:posterolateral bone graft fusion to the adjacent segments of the 30 patients whose adjacent segments consist mild degeneration after lumbar fusion and fixation.After an average of 41.3 months following,observe the situation of the adjacent segments and clinical symptoms recurred or not.Result:A total of 30 cases,5 adjacent segments' ROM disappeared,23 adjacent segments' ROM reduced under 10°,the 28 cases had no recurrence of low back pain and leg pain,and no degenerative phenomenon in the imagings of the adjacent segments.2 cases failed in fusion,the ROM increased,3 years after the operation,the clinical symptoms recurred and performed the second surgeries.Conclusion:Adjacent segment degeneration associated with lumbar surgery is one of the causes for recurrence of law back pain leg pain,especially when the adjacent segment consist mild degeneration,the lumbar fusion and fixation could accelerate the degeneration.Posterolateral bone graft fusion to the adjacent segment after lumbar fusion could stable the adjacent segment and reduce the ROM,it pretends the degeneration effectually and enhances the therapy. |
