Object:Type 2 diabeteses mellitus are important and dangerous factor of atherosclerotic vascular diseases.To research the relation and interrelation between CD40-CD40L signal path, sICAM-1, NO and the genesis and development of type 2 diabetic atherosclerotic diseases through detecting the concentration of soluble CD40 ligand(sCD40L),the soluble Intercellular Adhesion Molecule-1(sICAM-1),the nitric oxide(NO) and intima-media thickness of the common carotid artery in diabetes group,simple macroangiopathy group and control group,which will may may provided the new way of thinking for study the pathogenesy,early diagnosis,prevention and cures.Method:Research objects is divided into 3 groups:cotrol group 31 cases,simple macroangiopathy group 30 cases and diabetes group 100 cases, among them 45 of simple diabetes, 51 of the diabetes merges macroangiopa thy.To collect all research objects fasting elbow vein blood.Adopt double antibody sandwich enzyme linked immunosorbent assay ( ELISA) measure blood plasma sCD40L, blood plasma sICAM-1,nitrate reductase method measure the blood plasma NO, and the fasting blood glucose(FBG),blood lipid,hepatic function , kidney function , uric acid , glycosylated hemoglobin A1(HbA1c), the fasting insulin(FISI),and other biochemical index sign. The same period records the sex, age, height, weight,body mass index(BMI),waist-to-hip ratio(WHR),course and blood pressure and etc..Examine the carotid artery IMT.Carry on the data processing with the SPSS11.5 statistics software, data express with mean±standard deviation,multigroups comparison by analysis of variance(ANOVA),two group comparison by q test.two factors comparison by pearson linear correlation analysis,multiple factors use gradually linear regression analysis.Result:The blood plasma sCD40L concentration,sICAM-1 concentration ,carotid artery IMT increase obviously in DM group and more increase in DM combined with macroangiopathy.The blood plasma sCD40L concentration,sICAM-1 concentration in the DM combined with macroangiopathy increase obviously compare with simple DM group; blood plasma NO concentration is not obviously difference between DM group and normal matched control.The blood plasma sCD40L concentration is positive correlated to blood plasma sICAM-1 concentration and HbA1c and is negative correlatde to blood plasma NO concentration;Through multiple linear regression analysis we find that the blood plasma sCD40L,sICAM-1 concentration is closely correlate to carotid artery IMT of type 2 diabetes.Conclusion:(1)The increased expression of blood plasma sCD40L,sICAM-1 may be involved in the genesis of macroangiopathy in type 2 diabetes;(2)The increased expression of blood plasma sCD40L may be correlate to the increased expression of blood plasma sICAM-1;(3) The increased expression of blood plasma sCD40L may result in the endothelial injury,which will initiate the genesis of type 2 diabeteses earlier period atherosclerosis.
