| Objective: The aim of this study is to detect M.pneumoniae by quantitative enzyme-linked immunosorbent assay(ELISA) and Polymerase chain reaction(PCR), compare them with sensibility and correlation. meanwhile, explore epidemiology features and laboratory results in Soochow, to provide basic information for the early diagnosis and the rational therapy.Measurements: patients with acute respiratory tract infection on admission during 2005.10 to 2006.9 were enrolled and 2ml venous blood was adopted for 1851 patients, and 586 patients provided for paired serum as well as 151 parents'serum. Both MP IgM and IgG were detected by quant ELISA for diagnosis of acute infection. In addition, sputum which were detected by PCR were adopted in 1508 patients within 24 hours. 1355 patients were detected via ELISA and PCR.Results: (1) According to age distribution, the positive rate among which the numbers 1~3 months old, 4~6 months old, 7~12 months old, 1~3 years old, 4~6 years old, 7~14 years old are 2.48% , 4.87%, 14.00%, 31.26%, 35.07%, 70.54%. According to different seasons, the positive rate are 20.62%,27.74%,29.02%,12.89% from spring to winter.When it comes to different diseases, lobar pneumonia masculine gender rate was highest, next manifests suddenly for the asthma, respectively is 73.91% and 39.70%.The results of 151 examples of children- parent's serum antibody examination indicated there are of the linear correlation(P<0.01) between children's MP infection and the family member's. (2) B, C, D, E group of IgG are higher than the normal reference value scope the mean value, A and F group is lower than the normal reference value scope the mean value, however,with the exception C group, other several groups still in normal reference value scope; IgA, IgM of other groups are higher than normal reference of the age group's value scope except F group. CD4 of 1-6 month A and B two group is higher than other age groups. The CD4/CD8 value of F group is higher than other age groups, also obviously is higher than normal value 1.3; A, B, C, E group are lower than the normal value, D group approaches normal value 1.6. Compared lower respiratory infection of MP, RSV with no pathogen patients, wheezing rate of MP is 36.29%, RSV is 71.22%, and no pathogen patients is 25.88%, the wheezing rate of lower respiratory infection caused by MP, RSV are high (P<0.05). The positive rate of the paired serum antibody examination is higher than the single serum, the phlegm PCR examination is lowest; PCR and the ELISA uniformity is better(0.4<К<0.75).Conclusion: 1,Young children infected MP have the obvious epidemiology characteristic: MP infection rate Unceasingly increases along with the age growth, moreover, the incidence of infections caused by MP is also high in children aged less than 5 years; MP infection in autumn was more than other seasons;Otherwise,pneumonia isthe Most clinical manifestation. There is a a strong correlation between The children and the family member infection. 2,Young children infected MP were followed by immunity function disorder, displays for the humoral immunity hyperfunction, the cellular immunity function hypofunction. Meanwhile, MP infection is closely related to asthma occurrence development. 3,Sensitivity and specificity of the paired serum antibody examination are higher than the single serum, simultaneously unites the PCR examination, which can reduce the only one method examination limitation, It is very useful in clinic. |
