Objective The aim of this study was to investigate the expression of soluble triggering receptor expressed on myeloid cells-1(sTREM-1)in pleural effusions of various causes and to analyze the influential factors.Methods ELISA was used to measure the pleural fluid and peripheral blood concentration of sTREM-1 which come from 12 patients with infectious pleural effusion,31 patients with tuberculous pleural effusion,35 patients with neoplastic pleural effusion,and 15 patients with transudate.Meanwhile,cells in the pleural fluid and in the peripheral blood were counted and classified.Then the levels of sTREM-1 and other Parameters were entered into multiple linear regression.Results The concentration of sTREM-1 in pleural effusions was significantly higher than that in corresponding peripheral blood in all causes(P<0.05).The levels of sTREM-1 in both infectious pleural effusion and corresponding eripheral blood[2616.9(2096.8);38.1(28.1),pg/ml]were the highest in all groups.No significant differences of the sTREM-1 levels from pleural effusion and corresponding peripheral blood were observed between pure tuberculous pleural effusion[343.3(335.6);0.2(8.7),pg/ml]and tuberculous pleural effusion complicated with pneumonia[239.7(362.9);14.3(24.1),pg/ml](P>0.05),also between pure neoplastic pleural effusion and neoplastic pleural effusion complicated with pneumonia[595.0(1372.9);14.3(24.1),pg/ml]VS.[485.2 (1260.9);1.5(16.0),pg/ml](P>0.05).The main contribution factors of sTREM-1 in pleural effusion were LDH,glucose,granulocyte ratio,eukomonocyte ratio and nucleated cell counts in effusion.conclusions TREM-1 expression was increased in the disease of pleura.While the level of sTREM-1 in infectious pleural effusion was the highest in all causes. The concentration of sTREM-1 in pleural effusions may be a diagnosis index of bacterial infection.Whether or not complicated with pneumonia may not have obviously influence on the levels of sTREM-1 of pleural effusions and corresponding peripheral blood.The main contribution factors of sTREM-1 in pleural effusions were LDH,glucose,granulocyte ratio,leukomonocyte ratio and nucleated cell counts in effusion.
