Objective: To explore the clinical effects of titanium mesh and bone graft combined with anterior locking plate therapy on the multilevel cervical spondylosis. Methods: To analyze forty cases with multilevel cervical spondylosis, which was treated by titanium mesh and bone graft combined with anterior locking plate from June 2003 to June 2006. There were 23 males and 17 females, 56.3 years (ranged 39-73) were averaged. Lesion was involved 16 cases with single vertebrae and two intervertebraes, 19 cases with two vertebraes and three intervertebraes, 5 cases with three vertebraes and four intervertebraes. The curvatures of cervical spine with D numerical value were measured on the radiography of cervical spine between preoperation and postoperation as well as late follow up period, the neurological functions scaled by JOA score were observed respectively in preoperation,postoperation and late follow up period, the data was statistically analyzed. Results: All cases were regularly followed up, during the followed up time ranged from 6 months to 36 months(the mean 15.8 months). 69 cervical vertebraes and 109 intervertebraes were excised. Fusion were reached in all cases during the mean 9.3 months(ranged 6-13 months). The scores of JOA were increased from preoperative 9.5±1.4 scores to postoperative 13.2±1.7 scores,late follow up scores of JOA were 13.7±1.3 scores, which were not obviously lost, the curvature of cervical spine with D numerical value raised from preoperative 4.7±1.0 mm to postoperative 9.9±1.2 mm, D numerical value of late follow up were 9.6±1.2 mm, which were not obviously lost, the difference was found to be statistical significance(P<0.05). 7 cases of titanium mesh were subsided, the rate subsidence of titanium mesh was 17.5%(7/40 cases). Conclusion: Titanium mesh and bone graft combined with anterior locking plate therapy on the multilevel cervical spondylosis can obtain a good effect in short period. It's one of the safety and effective bone graft method, but the complications of subsidence of titanium mesh should be highlighted.
