To evaluate the efficacy of multislice helical CT perfusion imaging in differential diagnosis of solitary pulmonary nodules. Materials and Methods:Eighty two patients with solitary pulmonary nodules or masses proved by clinical examination or pathology underwent CT perfusion imaging.They were classified into three groups according to the pathological results. (malignant nodules in 55 cases, inflammtory nodules in 20 cases and benign nodlules in 7 cases) . All patients were performed perfusion scan and helical scan by multislice helical CT.Their hemodynamic parameters including blood flow(BF), blood volume(BV), mean transit time(MTT), permeability surface (PS) were measured in region of interest. With the CT perfusion body tumor software,the parameters of CT perfusion including BF, BV,MTT,PS,were analyzed in region of interest statistically.Results:These parameter values in the malignant nodules and the inflammtory nodules were all higher than those in the pulmonary benign nodules.The differences of BV and BV were statistically significant among the three groups (P<0.01) ,No statistic significant difference in MTT was found among the three groups(p>0.05).There were statistically differences in the PS between the benign nodules and the malignant nodules or the inflammtory nodules(p<0.05),but no statistical differences between the malignant nodules or the inflammtory nodules (p>0.05).When BF>230 ml/(100mg*min)was set as the diagnostic threshold,the sensitivity,specificity,positivepredict value and negative predict value were87.3%,81.5% ,90.5% ,75.9%respectively.When BV>800ml/100mg was set as the diagnostic threshold,the sensitivity. specificity,positive predict value,negative predict value and were 90.9% ,74.1% ,90.6% ,80.0% respectively.Conclusions:Multislice helical CT perfusion imaging could provide quantitative hemodynamic information to help the diagnosis of solitary pulmonary nodules.It is helpful to differentiate lung cancer from pulmonary benign masses with multislice helical CT perfusion imaging.
