Objective: To investigate the expressions of platelet-derived CD40,CD40 ligand(CD40L) ,serum concentrations of sCD40,sCD40L and plasma concentrations of CD62P in patients with lung cancer, and to evaluate the clinical significance of CD40/CD40L in lung cancer. The antithrombinâ…¢levels of all participants were also evaluated in order to illuminate the role of coagulation system in the CD40/CD40L pathway.Methods: Platelets, plasma and serums were isolated from 85 patients with lung cancer and 25 normal subjects. The expression rates of platelet-derived CD40,CD40L,CD62P were measured by flow cytometry before and after adenosine diphosphate(ADP) inducing in vitro. Meanwhile platelets were isolated from 6 participants of three groups respectively, the platelets were stimulated with ADP in vitro for the indicated times, and the levels of CD40,CD40L,CD62P expression were assessed by flow cytometry. The serum concentrations of soluble CD40(sCD40),soluble CD40 ligand(sCD40L) and the plasma concentrations of CD62P were measured by enzyme linked immunosorbent assay. The antithrombinâ…¢levels of all participants were evaluated at the same time.Results: (1) The expression of platelet-derived CD40,CD40L,CD62P were significantly increased in patients with lung cancer as compared with normal volunteers before induced by ADP(P<0.01). After induced by ADP, the expression of CD40,CD62P in patients were significantly higher han those of normal volunteer(sP<0.01),but the expression of platelet-derived CD40L showed no statistically significant difference between patients and normal volunteers. (2) Before and after induced by ADP, the expression of platelet-derived CD40,CD40L,CD62P showed no statistically significant difference among all pathological types of patients with lung cance(rP>0.05). (3) Before and after induced by ADP, The elevation of platelet-derived CD40L,CD62P were correlated with progression of lung cancer. The expression of platelet-derived CD40 showed no statistically significant difference among all stages of patients with lung cancer(P>0.05). (4) Before and after induced by ADP, The expression of platelet-derived CD40L and CD62P were higher in the patients with metastasis than those of without metastasis(P<0.05). (5)The levels of CD40L,CD62P were significantly positively correlated with inducing time extending(r=0.521, P<0.01). (6) The serum of sCD40,sCD40L and the plasma of CD62P levels in patients with lung cancer was significantly higher than that of normal volunteers. (7) The concentrations of sCD40,sCD40L,CD62P showed no statistically significant difference among all pathological types of patients with lung cancer(P>0.05). (8) The levels of sCD40,sCD40L,CD62P elevated with progression of lung cancer. (9) The concentrations of sCD40,sCD40L,CD62P were higher in the patients with metastasis than those of without metastasis(P<0.05). (10) The levels of sCD40L,CD62P in patients with lung cancer decreased when two weeks after surgery, furthermore the levels of sCD40L,CD62P were positively correlated(r=0.300,P<0.05). The concentration of sCD40 after surgery showed no discrimination with that before surgery(P>0.05). (11)The level of AT-â…¢in patients with lung cancer was significantly lower than that of normal volunteers(P<0.01),but their platelet counts showed no statistically significant difference with normal volunteers(P>0.05). (12)Correlation analysis among sCD40L levels and other variables showed the presence of a direct correlation with plasma CD62P( r=0.388,P<0.05),plasma AT-â…¢levels (r=-0.421,P<0.01).Furthermore, CD62P levels significantly correlated with AT-â…¢levels( r=-0.256,P<0.05). (13) Therefore, to further analyze the relationship between sCD40L and clinical variables of lung cancer, a multiple regression analysis including age, sex, tumor size, plasma CD62P, and AT-â…¢levels was carried out. The final model obtained by regression analysis revealed that only plasma CD62P(coefficien[tSE]=0.422, P<0.01)levels were independently related to sCD40L (r2 for entire model =0.635, P<0.01).Conclusions: (1)These results suggested that there were high levels expression of platelet-derived CD40L,CD62P,serum concentrations of sCD40,sCD40L and plasma concentrations of CD62P in patients with lung cancer. Therefore the levels of them were correlated with the stages of lung cancer. The system of platelet-derived CD40/CD40L might have a potent effect in pathogenesis of lung cancer. (2)The expression of CD40L had a positive relationship with the expression of CD62P indicated that CD40L could be one of biomarkers of platelet activation. Platelet activation might exert great influence during the progression of lung cancer. (3)The levels of coagulation activity in patients with lung cancer were increased ,however the elevation of sCD40L might result from platelet activation but not coagulation activation.(4) Measurement of platelet-derived CD40L,CD62P may therefore provide a tool for monitoring the clinical course of lung cancer.(5) Inhibition of platelet activation could be another therapy of asthma control, and CD40/CD40L might be an important target molecule on platelets.
