The Express Of OPN And VEGF In Pancreatic Carcinoma And Its Relationship With Tumor Infiltration,Metastasis And Prognosis

Object:To evaluate the expression of OPN and VEGFR-3 in pancreatic carcinoma and its relationship with tumor infiltration,metastasis and prognosisMethod: the specimens were taken from 60 patient with pancreatic carcinoma in An Hui Medical Affricated Hospital from January 2000 to April 2004. Among them, the ration of sex man and woman was 39:21;the age varied from 28 to 77 years old and the median age was 56; 22 cases had lymph node metastasis and 38 without. The radical operation were taken to all cases and lymphadenectomy were used.additional,15 pancreatic specimens biopsied were taken from the distance≧5cm off the pancreatic cancer specimen as control.using SP immunohistochemistry,we studied the expression of OPN and VEGFR-3 in pancreatic carcinoma and its relationship with tumou infiltration metastasis and prognosis.SPSS 13.0 was used to analyze the results.Result: 1.the expression of OPN in pancreatic carcinoma was significantly higher than that in control (68.3%vs26.7%,P < 0.01) ,Immunohistological staining with OPN specific polyclonal antibody showed cytoplasmic and membranous staining in the carcinoma cell.2.the positive expression of VEGFR-3 is chiefly located in cytomembrane and cytoplasm of lymph vessel endothelial cells.The VEGFR-3 positive lymph vessel were main seen in tumor periphery. Those lymph vessel have large lumen ,thin wall and irregular shape;furthermore,some carcinoma cells are involved in VEGFR-3 positive lymph vessels.3.By T test,we found that the number of VEGFR-3 positive lymph vessels in OPN positive group was higher than that in OPN negative group (16.28±5.15vs 9.75±2.82,p<0.01)4.The positive expression of OPN in lymph node metastasis group is higher than that in no lymph node metastasis group (85.7%VS58.9% P〈0.05 ); By logistic regression analysis, we found that OPN expression and the number of VEGFR-3 positive lymph vessels are independent factors that influence lymph node metastasis .5.By Kaplan-meier and Cox regression model analysis ,OPN expression and TNM termination are found to be independent factor for patients'prognosis.Conclusion:1.OPN is mainly expressed in cytoplasmic and membranous of pancreatic carcinoma ,and most are overexpression;the positive expression of VEGFR-3 is chiefly located in cytomembrane and cytoplasm of lymph vessel endothelial cells.The VEGFR-3 positive lymph vessel were main seen in tumor periphery .2.The overexpression of OPN and TNM termination are independent factors for patients'prognosis. The expression of OPN has a relationship with the survival time of pancreatic cacinoma's patients.
