The Assessment In Cardiac Function Of Graves Disease Before And After Treatment With 131-Iodine By Nuclear Imaging

Objective To investigate the value of radionuclide ventriculography in assessment the change and clinical significance of ventricular function in hyperthyroid patients before and after iodine therapy.Methods The 30 patients with Graves disease who were euthyroid after 131-iodine therapy is group 1,Patients with any known cardiac disease were excluded.The normal control group included 10 people is group 2.The serum levels of the thyroid hormones(FT₃,FT₄,),h-TSH and TRAb were measured and the heart function parameters(EF,PER,TPER,PFR,TPFR)and phase analysis was obtained by radionuclide ventriculography in both two groups.Results Before therapy,The serum TH in group 1 were higher than the group 2,and h-TSH in group 1 was lower than the group 2.6 months after iodine therapy,the inhere clinical symptoms and sign disappeared,FT₃,FT₄ dropped down to the normal range.At the same time,h-TSH rose up(Comparing to the group before therapy,P＜0.01;Comparing to the contrast group,P＞0.05). Before therapy,comparing the group 1 to the group 2:there were statistical significance in the increasing of EF,PER of right ventricle and PER of both ventricle,and no statistical significance in the EF of left ventricle and TPER of both ventricle.There were statistical significance in the increasing of PFR,TPFR and phase analysis in both ventricle.In pair of comparison between before and after 131-iodine therapy in group 1,each parameter of the ventricular function was all higher except the EF of LV and the TPER of both ventriculars,before therapy than which after 6 month's 131-iodine therapy,((P less than 0.01).In compare with between euthyroid after therapy and the group 2,each parameter of ventricular function was higher in the former than in the latter,but there were not statistical significance.Conclusions Before complicating Hyperthyroid heart disease,the ventricular systolic and diastolic performance in patients with hyperthyroid were already increased compensately.The change of diastolic function is obvious. And the abnormality in right ventricle is more common than the other.Further more,the concordant of the heart decreased obviously.All of above can get amelioration in Graves disease patients after being therapied by 131-iodine.The radionuclide ventriculography is an effective and sensitive method to estimate these manifestation.
