Objective: To determine the relation both soluble CD40 ligand(sCD40L), myeloperoxidase(MPO), high sensitivity C reactive protein(hs-CRP) and diameter of primary coronary arteries and disease severity of coronary.Methods: We randomly select 85 patients who have been all recepted coronary angiography,and divided into four groups according to stenosis of primary coronary diameter: group with no stenosis: normal vessel or stenosis diameter≤50%(n=24),group with slight stenosis range: 50%90%(n=30). At the same time, the subjects are divided into AMI(acute myocardial infarction) group(n=14),UAP(unstable angina pectoris) group(n=37),SAP(stable angina pectoris)group(n=15)and normal control group(n=19) according to clinical severity. sCD40L,MPO and hs-CRP must be assessed respectively with ELISA,colorimetry and turbidimetry.Results:⑴The serum sCD40L of group with serious stenosis (15.84±4.84ng∕ml)is significantly higher than other group (P<0.05~0.001), the other groups have no statistic differences. In the face of disease severity, the serum sCD40L in AMI(16.42±4.66ng∕ml) and in UAP(13.50±5.29 ng∕ml) is significantly higher than in SAP(9.22±3.65 ng∕ml) and control group(8.66±2.90ng∕ml).⑵The serum MPO have statistic differences between every two group in face of all stenosis group (F=25.95, P<0.001) ,the serum MPO in AMI(156.61±33.63 u∕L) is significantly higher than in UAP(92.06±42.11u∕L),in SAP(80.02±20.28 u∕L) and in control group(52.33±14.32 u∕L), MPO in UAP is have no statistic differences in SAP(P=0.35).⑶As for hs-CRP in face of vessel stenosis, besides serum hs-CRP in group with serious stenosis is slightly higher than in control group(3.61±0.64 versus 1.41±0.55,P<0.05), have no statistic differences between the other groups. As for hs-CRP in face of clinical severity, hs-CRP in AMI(5.74±1.09mg∕L) is significantly higher than in UAP(1.91±0.72mg∕L,P<0.001),in SAP(2.61±0.60 mg∕L,P<0.01) and in control group(1.47±0.47mg∕L, P<0.001), have no statistic differences between the other groups (P>0.05).⑷There is negative correlation between sCD40L and HDL-C(r=-0.32,P<0.01), there is a slight positive correlation between sCD40L and ANC(r=0.37,P<0.01), have no correlation between sCD40L and MPO,hs-CRP. There is a positive correlation between MPO and hs-CRP,ANC (r=0.27,P<0.05;r=0.48,P<0.01), have no correlation between MPO and blood-fat.Conclusions: The level of serum MPO have statistic differences among all stenosis groups of coronary heart disease, but sCD40L and hs-CRP have no statistic differences; The level of serum sCD40L have statistic differences in face of clinical severity, but MPO and hs-CRP have no statistic differences.
