| Objective Understanding the present status of diet therapeutic compliance and the effect of health education among patients with diabetes mellitus, detecting influential factors of compliance and metabolic control ,searching for more effective methods of health education, providing theory evidence and scientific guidance for improving integrate control of diabetes mellitus.Methods 270 patients from the second and the third clinical hospital of jilin university and three residents' committees of Changchun were investigated by self-made questionnaire and detected body height , body mass , FPG, RPG, blood-fat and GHbA1c ; 270 patients were randomly divided into education intervention group 123 cases(n=135,missing 12 cases)and control group 113cases(n=135,missing 22cases). The patients in the control group received the routine diabetic treatment and general diabetes education including diet, exercise, drugs, self-monitoring, insulin therapy, prevention of complications, 60 minutes each time; The patients in the intervention group received individual target education and behavior intervention and were followed up by every month. Specific approach: the total goal of diet control will be divided into a number of goals of the stage easily to achieve, and gradually strengthen diet therapy. Patients were followed up each time with the correct diet problems based on diary, and on this basis new educational goals were developed. After their final visit, all the patients in the two groups were investigated once again and detected body height , body mass, FPG, RPG, blood-fat and GHbA1c.Results 1. The study showed that 270 questionnaires in compliance good (Score≥60) accounted for 8.89 percent, the compliance of the middle (Score≥48) accounted for 18.11 percent, low compliance (Score <48) accounted for 73.00 percent. 2. Diabetic diet treatment-related knowledge survey, which have a good degree (greater than 16 correct answers few issues) accounted for 28.10 percent, middle degree (number of correct answers than 8 issues) accounted for 37.83 percent, low degree (less than 8 correct answers few issues) accounted for 34.07 percent. 3. Single factor analysis showed that age, occupation, education level, income, sources of medical expenses five variables affect the compliance of patients with diabetes (p <0.05). 4. Multivariate logistic analysis showed that age, education level, income ,sources of medical expenses ,occupation were the risk factors, in other words, these factors were most effective for compliance. 5. The compliance of diet therapy and scores of diet treatment-related knowledge and FPG, RPG, GHbA1c in the intervention group improved compared with control group, the difference was significant in the study(P<0.05) but as to BMI ,TC,TG,HDL-C,LDL-C, there were no statistical difference between groups(P>0.05).Conclusion 1.The total level of diet therapeutic compliance was not well among patients with diabetes mellitus in this study 2. The main influential factors of compliance for patients with diabetes mellitus were age, education level, income, sources of expense and occupation. 3. Individual target education and behavior intervention on the patients with Diabetes mellitus offer appropriate guidance in accordance with the differences in occupation, education and ability which may be better than traditional health education on improving diet therapy compliance and beneficial for metabolic control. |
