Objective To evaluate 1H Magnetic resonance spectroscopy (1H-MRS) in the differentiation of necrosis and recurrence of brain gliomas after radiotherapy. Methods 27cases of patients sufered from brain gliomas were selected, all of them had the indication of necrosis or recurrence after radiotherapy. In which, there were 12male and 15 female, the age ranged from 18 to 66 years. 1H-MRS were performed in all 27 cases. Results There were 14 cases of recurrence of brain gliomas, as well as 9 cases of necrosis after radiotherapy, 4 cases of recurrence of brain gliomas and necrosis after radiotherapy. The imaging manifestation of gliomas recurrence appeared as follow: The rise of peak of choline, the fall of peak of N-acetyiaspatate, as well as the sharp rise of peak of lactate, in which was mainly composed of lipid were present in 1HMRS. 2.The imaging manifestation of gliomas necrosis appeared as follow: all of peaks of choline, N-acetylaspartate, and creatine were descended. Conclusion 1HMRS is a helpful method to detemine recurence or necrosis of brain neoplasm after radiotherapy.
