Objective To study the Middle term curative effect of'conventional discectomy'and'conventional discectomy with posterior internal fixators'. Method To review 440 cases of prolapse of lumbar intervertebral disc treated with four surgica1 methods:vertebral plate windowing discectomy (group A),hemiinterbral plate discectomy (group B),holovertebral plate discectomy (group C) and all of these approaches with internal fixators(group D), the clinical results of different operation apporches were evaluated according to Oswestry disablibility index method . Result The excellent and good rates of four methods were 88.76%?83.61%?77.27%?90.54% respectively, There is no differences between group D and A , B(P>0.05) ,the difference between group D and C was statistically significant (P<0.05); the excellent and good rates of four methods in single segment were 89.93%?85.58%?78.57%?90.77% respectively, the excellent and good rates of four methods in double segments were 81.48%?76.47%?70.00% ? 87.50% respectively, There is no differences between group D and A,B,C(P>0.05). Conclusion The conventional discectomy with posterior internal fixators placed a good ground for posterior decompression and fusion of lumbar disc herniation,either single segment or double segments the curative effect could not be improved with posterior internal fixators.The conventional discectomy was still safe and effective method to treat lumbar disc herniation.
