| Objectives: the objectives are to study the feasibility of introducing Objective-structured Clinical Examination (OSCE) into the test for clinical ability evaluation, to design and apply the testing project of OSCE to the test for clinical ability evaluation, and to research the impact of OSCE on the clinical ability of trainees and the quanlity of clinical practice.Approaches: Design the testing project and grading standard of OSCE in the test for clinical ability evaluation.Test the 96 trainees graduate in 2001, who are undergraduates major in clinical medicine with this method. Analyze the effectiveness and reliability of the testing method. And analyze the discrimination and difficulty of the testing content. Distribute to the employers of 2001, 2000 and 1999 graduates questionaires on investigating the clinical ability of them. Distribute to the trainers of 2001, 2000 and 1999 graduates questionaires on investigating their general behavior. Distribute to 2001 graduates questionaires on investigating the impact of the testing method of OSCE on their individual development.Results: 1. The testing project of OSCE in the test for clinical ability evaluation is obviously related to the basic theories, professional theories and comprehensive testing results that the graduated have experienced at school within four years (P<0.001). 2. The Kappa readings of the testing project of OSCE at stations vary from 0.78 to 0.92 and that among testers is 0.76-0.91. The degree of difficulty of testing contents (p) is between 0.66 and 0.84 and the difference index (r) is 0.14-0.41. 3. There is an obvious difference in the rates of good evaluation for the practicing students between in Grade 2001 and the former two grades. (p<0.01) 4. The evaluation on clinic ability of practicing students in Grade 2001 is great different from that of the former two grades students. 5. An obvious difference in graduation results can be seen between the students in grade 2001 and the former grades students. (p<0.05)Conclusion: The testing project of OSCE in the test for clinical ability evaluation is a successful set of testing method for evaluation, which has a strong operationality and a highly reliability. In addition, the way of OSCE is included in the whole practicing procedure and closely related to the practicing plan. It enables students to maintain active in the whole practice and the aim for student's autonomic learning has been actually reached, which promotes such comprehensive abilities as clinic ones and quanlity of clinical practice. |
