The Exploration For The The Correlation Of Intervertebrae Instability With Cervical Vertigo And The Effect Of Taking Traditional Chinese Medicine Based On Different TCM Syndrome

The first section: The value of The Cervical Lateral Projection in The Extension and Flexion for Cervical VertigoObjective:To Evaluate the Value Of The Cervical Lateral Projection In The Extension and Flexion for Cervical Vertigo,explore the correlatioin of intervertebrae instability with cervical vertigo.Method:62 Patients Of Cervical Vertigo were Examined and Treated in Our Hospital from January to December in 2007,They Were named patient Group.45 Patients Without Cervical Vertigo Is Collected,they were named normal group.They Also Accepted The Cervical Lateral Projection in The Extension And Flexion.The Lenghui' s standard Was quoted to Evaluate The Stability Of The Cervical Motion Unit.Calculate The Ratio of Instability In The Two Terms,Measure the Angle Which Combined with the Vertebrae' s Posterior Margin in the lateral Projection.Analyze the Difference of The Two Groups.Result:Instability did not occured in the Normal Group,The average Angle combined with The Vertebrae' s Posterior Margin is 3.61±0.43°;44 Patients occurred Instability in the Cervical Vertigo,The Ratio of Occurring Instability is 70.97%,The average Angle Combined with The Vertebrae' s posterior margin is 6.86±0.93°.The Ratio Of Instability Occurred in Cervical Vertigo Group Was more than The Ratio in The Normal Group(P＜0.01).The Average Angle Combined with the Vertebrae's posterior margin in the cervical vertigo group Was more than the Normal Group(P＜0.05).Conclusion:The Cervical Lateral Projection in the Extension and Flexion is Simple,Economic,Useful,It can find Cervical Saggital Instability in lower cervical,Has great significance in Cervical Vertigo.The second section: Exploration of disease character and TCM syndrome distribution character of cervical vertigo in different ageObjective:To Explore The disease character and TCM syndrome distribution character of cervical vertigo in different age.Replenish the cervical vertigo's diagnosis standard.Method:62 Patients of Cervical Vertigo were Examined and Treated in our Hospital from January to December in 2007,They are divided into two groups (Below 50 years old and Above 50 years old),they are namedⅠgroup andⅡgroup.All of the Patients accepted the Disease History Recording,Physical Examination,X-ray,MRI,MRA,TCD Examination,They are Diagonized by two experienced TCM doctors.I make a table which consist of the Factors related with the Cervical Vertigo based on the previous research result.Recording the patients' information by it.Then analyze the difference between the two groups.Result:1.The character of syndrome in the cervical vertigo.Vertigo:The occurring ratio in the younger group is less than the older group(P＜0.01);Dizziness: The occurring ratio in the younger group is more than the older group(P＜0.05);Neck shoulder occipital pain:it is the commonest complaint in the two groups,no significant difference between the two groups(P＞0.05);Tinnitus or deafness:The occurring ratio in the younger group is less than the older group(P＜0.01);Vomitting:The occurring ratio in the younger group is less than the older group(P＜0.01).2.The character of physical syndrom in the cervical vertigo.Rotation sign and extension flexion sign:The positive ratio had no significant difference between the two groups(P＞0.05);The tenderness in the neck shoulder occipital: The occurring ratio in the younger group is less than the older group(P＜ 0.05).3.The character of imageology in the cervical vertigo.The abnormal curvature:The occurring ratio in the younger group is less than the older group (P＜0.05);The instability:No significant difference between the two groups (P＞0.05);The disc degeneration:The occurring ratio in the younger group is less than the older group(P＜0.01);The disc herniation:No significant difference between the two groups(P＞0.05);Abnormality in MRA:The occurring ratio in the younger group is less than the older group(P＜0.01);In the older group,The occuring ratio of rapid speed of vertebrae artery blood flow in the TCD is less than the younger group(P＜0.01),The occuring ratio of slow speed of vertebrae artery blood flow in the TCD is more than the younger group (P＜0.01).4.The distributing character of TCM syndrome.In the younger group,The Sthenia syndrome is most(P＜0.01),And the phlegm obstruction syndrome is most in the sthenia syndrome(P＜0.01);In the older group,The sthenia and asthenia syndrome is most(P＜0.01),And the weakness of kidney and live with phlegm obstruction syndrome is most in the the sthenia and asthenia syndrome(P＜0.01).Conclusion:1.Clinical manifestation in the different age groups the dizziness is common complaint in the below 50 years old group;The common complaint in the above 50 years old group is frequent,long-time,severe dizziness or vertigo with vomiting or tinnitus or deafness.Neck shoulder occipital pain is common complaint for cervical vertigo patients.2.Neck rotation and extension flexion sign are important physical examination.3.Image:X-ray Cervical instability,Cervical Hypertrophy,Disc degeneration,Proplased intervertebrae disc are common manisfestations.TCD is helpful to diagnosis,MRA and MRI can show the compression and abnormality of vertebrae artery. The third section:The exploration for the effect of taking different Traditional chinese medicine based on differnt TCM syndrom to Oervical VertigoObjective:To Evaluate the effec Of taking traditional chinese medicine to cervical vertigo.Method:62 Patients Of Cervical Vertigo who had been in Our Hospital from January to December in 2007,They Were divided to two groups at random,named the taking TCM group and control group.There were 31 patients in every group.The patients in the control group obtain the integrated therapy wbich was consisted of medicine,physical and rest.The patients in the taking TCM group obtain the integrated therapy assisting with taking the different traditional chinese medicine based on different TCM syndrome,we follow up in the posttherapy 3^rd,6^thmonth.To analyze the effect of different therapy by the function Score,Recurrence.Result:1 The character of TCM syndrome distrbuting:in the 62 patients,31 patient were the phlegm obstruction syndrome;26 patients were the weakness of kidney and live with phlegm obstruction syndrome;5 patients were the other syndrome.2 the evaluation of the effect of taking TCM to cervical vertigo:Two group also can take good effect in the short-term follow-up,taking TCM can improve the effcet(P＜0.01),and decrease the recurrence rate。Conclusion:taking TCM can improve the effect of the integrated therapy to the cervical vertigo.Decreased the recurrent rate in the short-term follow-up.
