The Effects Of Ambroxol With Different Dose To Inflammatory Cell Factor Of Rabbits On One-Lung Ventilation

Objecfive:one-lung ventilation was a usual technique on chest surgical anaesthesia.Many factors induced a trail of lung injuries and complications of lung in one-lung ventilation,including tissue of lung ischemic,hypoxia,machine drag after recruitment, ischemia-reperfusion injury,trauma of surgery and so on.IL-6,IL-8,TNF-αare important cytokine on acute lung injury.To study the changes of proinflammatory cytokine and evaluate how injury of lung in one-lung ventilation,we select them as monitoring indicator. This study aimed to observe the therapeutic efficacy of ambroxol on one-lung ventilation(OLV),and moreover the therapeutic efficacy of the ambroxol with different dose.Methods sixty-seven domestic rabbits were randomly divided into four groups:control group of two-lung ventilation(group A n=18), control group of OLV(group B n=16),ambroxol(5 mg/kg)treatment group(group C=15),ambroxol(15 mg/kg)treatment group(group D n=18). Blood specimens of B,C and D were taken at the point of pre-anaesthesia,60,120mins after one-lung ventilation and60,120mins after two-lung ventilation;And blood specimens of A were taken at the point of pre-anaesthesia,60,120,180,240mins after two-lung ventilation respectively.Results:1.Pathology of lung tissueThe rabbits' lungs of group A were light pink,their surfaces were smooth,full of elasticity,without hemorrhagic spots.The lungs of group B,C and D were of tumescence and hyperaemia.Lots of hemorrhagic spots were on their surfaces and their sections were loose and fragile,with much exudation.The lungs of group C and D were almost identical on the whole.Their damage is less serious than group A.HE staining on the lung tissue:Through observing HEstaining on the lung tissue,we can find that the constitution of alveolus of group A was normal and clear.The wall of alveolus was damaged,the space of lung broaden in group B,C and D,The changes of them focused on haemorrhagia,serous effusion, edema,inflammatory cell infiltration in alveolar space;but all of the histological changes in group C and D were not as serious as in group B.2.The expression of IL-6,LI-8,TNF-αThe comparision of group and pre-anaesthesia:The changes of group A was not obvious;the expression of group B,Cand D increase obviously(P＜0.01).The comparision of each group:group B,Cand D is much higher than group A(P＜0.01);group B is much higher than group C,D(P＜0.01), there is no significant difference between group C and D(P＞0.05)Conclusion:The ambroxol can better control the rise of the inflammation factors in OLV,so that it has some effect in preventing and curing the lung injury induced by OLV.When the dose of ambroxol increase to an effective dose,the curative effect might be related to the dose of ambroxol but doubling dose might not bring doubling effect.
