Objective:To investigate the appropriate time for expectant management and the time and way of pregnant termination in early onset severe preeclampsia.Methods:Condition of onset and pregnancy outcome of 86 patients with early onset severe pre-eclampsia admitted to our hospital from Jan 2003 to April 2008 were retrospectively investigated.Patients were assigned to three groups: Group A(gestation week<28 weeks),Group B(28 weeks≤gestation week<32 weeks),and Group C(32 weeks≤gestation week<34 weeks).Results:No statistically significant differences in complication incidence were seen between three groups,The differences in rates of perinatal mortality and neonatal asphyxia of the three groups was statistically significant(p<0.05). Expectant management in Group B was significantly longer than that in other groups(p<0.05).Most cases were delivered by the cesarean section in each group.Conclusion:The maternal neonatal prognosis was seriously influenced by the early onset severe preeclampsia,and the cases received expectant management must be stringently selected.Expectant management should be taken in prudence in pregnant women with onset before 28 weeks gestation,while positive expectant treatment under close monitor for those with onset during 28~31+6weeks,can prolong the gestation age and improve the outcome of perinatal infant.Cesarean section turn out to be the first choice to terminate pregnancy.
