OBJECTIVES: To appraise the value of mediastinoscopy in diagnosis of thoracic diseases and staging of lung cancer.METHODS: From August 1980 to October 2005, 295 cases accepted mediastinoscopy with biopsy of neoplasm and peritracheal lymph nodes through pretracheal interstice. 23 of them were given 1 eftanteriormediastinoscopy .The other 272 were given standard cervical mediastinoscopy.RESULTS: Operation time was 64. 2±21. 5min, amount of bleeding was45. 6±26. 8ml, without death after operation.Mediastinoscopy was an more effectivemethod for diagnosing mediastinal diseases than CT . Checking the lymph nodes in the mediastinium of lung cancer and sarcoidosis, mediastinoscopy was better than CT . The number of the lymph nodes in mediastinoscopy for diagnosis was different,and result was significantly different also.There was no different between TV- mediastinoscopy and common mediastinoscopyCONLUSIONS: Mediastinoscopy is an effective method for diagnosing diseases involving mediastinal lymph nodes, especially shoud be the first choice for diagnosing or staging lung cancer with enlarged mediastihal lymph nodes or those with mediastinal tumor. Sarcoidosis and lymphoid tuberculosis could be given timely treatment in diagnosis of mediastinal disease. It is a highly efficient and safety inspection.
