| Objective: Eustachian tube is one of the most complex organizational structures in human. Its anatomic location is concealment and the physiological function have not been fully understood . Eustachian tube dysfunction is more common and the secretory otitis media which is induced by pharynx drum tube dysfunction is the main reason for hearing loss in children . In this pape , we have analyzed the inspection results of the pure tone zudiometry , acoustic immitance and electron epipharyngoscope of the patients who may exist of the Eustachian tube dysfunction . By this , we have made clear the clinical audiology appearance of the Eustachian tube dysfunction and the morphologic characteristics of the ostium pharyngeyum tubae auditoriae which may provide a theretical reference for further exploring the Eustachian tube dysfunction in the pathogenesis and clinical accurate and timely diagnosis and treatment .Methods: 15 children patients with chronic rhinitis , 20 children patients with chronic sinusitis , 20 children with adenoidal hypertrophy , 20 children with SOM , 20 adult patients with chroni sinrsitis , 20 adult patients with nasal polyps , 20 adult patients with allergic rhinitis , 20 adult patients with SOM were given conventional pure tone audiometry , the acoustic-testing and electronic nasopharyngeal examination , 30 children and 50 adults with no age and gender differences in the health of populations were compared . To record relevant data for statistical analysis .Results:In PTT examination , children with chronic sinrsitis and adenoidal hypertrophy have a certain degree of reducing hearing , adults with chroni sinusitis , nasal polyos and allergic rhinitis also have an impact on the hearing and there were no significant differences between three groups . In acoustic immitance test , children with chronic sinrsitis and adenoidal hypertrophy , adults with chroni sinusitis , nasal polyos and allergic rhinitis all will due to ETDF to tympanic graph abnomal , middle ear pressure lessening . To study the form of ostium pharyngeum tubae aditoriae , we can know that In adult patient group and the control group , the ratio of triangle pharynx port is most in normal control groups . the ratio of crevice form pharynx port is most in case groups .The difference of oval-shap and flaring were not statistically significant.In children patient group and the control group , there's no significant differences between these four shapes . Sinrstitis group of children and adults sinusitis comparison group had a significant difference , children sinrsitis patients are more likely than adults to have lesions around the mouth of the eustachian tube . After surgery the hearing and middle ear function of children adenoidal hypertrophy and adult patients with nasal polyps have significant change . The ratio of all pharynx port in children adenoidal hypertrophy who were given surgery there is no significant change , The pharynx port of adult patient with nasal polyps after surgery are getting better .Conclusion:①Common nasal diseases such as chronic sinusitis, children adenoidal hypertrophy , nasal polyos , allergic rhinitis , etc may have a certain degree of influence to hearing , these patients should be given routine hearing examination when visiting to know the states of ET's function .②Children adenoidal hypertrophy , nasal disease and adult patient with chroni sinusitis , nasal polyos and allergic rhinitis change the normal physiology of nose lead to ETDF to the incidence of middle ear disease is significantlt increased than normal .③The different pathologyic change is one of important factors in SOM ; The part sharp is close relative with adult SOM .④Surgery coulid improve the hearing and middle ear functiong of children with adenoidal hypertrophy . So , we should adopt positive operation on them , lest missing the best therapy opportuneity .⑤Surgery coulid improve the hearing and middle ear functiong of adult with nasal polyos , the ports are getting better . So , we should adopt positive operation on them , improving the survival quality . |
