| Objective:To investigate the protective role of selective cyclooxygenase-2(cox-2) inhibitor (celecoxib) in the intestinal mucosal barrier injury of severe acute panereatitis.Methods:Seventy-two male Wistar rats weighing 230-260g were randomly divided into sham-operation(SO) group(n=24),SAP group(n=24),SAP treated with selective cyclooxygenase-2(cox-2) inhibitor(TW)group(n=24).Each group was divided into three small groups as the phases of 3h,6h,12h.Each small group was eight rats.SAP model was established in rats by injecting 5%sodium taurocholate into biliopancreatic duct.The changes of plasma endotoxion,DAO,D-lactate,TNF-α,IL-6and bacterial translocation were determined at 3,6and12 hour after operation.The intestinal lumen mucosa was examined pathologically at 3,6 and 12 hour after operation.Cyclooxygenase-2 expression in the intestinal mucosa was detected with immunohistochemistry at 3,6 and 12hours after operation.Results:The bacterial translocation,plasma endotoxin,D-lactate,TNF-α,IL-6 level in SAP group increased significantly compared with SO group(P<0.05).The bacterial translocation,plasma endotoxin,D-lactate,TNF-αand IL-6 level in TW group decreased significantly compared with SAP group(P<0.05).Few cyclooxygenase-2 was expressed in intestinal mucosa of SO group.More cyclooxygenase-2 was expressed in SAP group.The degree of cyclooxygenase-2 decreased in TW group compared with SAP group(P<0.05).Conclusion:The function of intestinal mucosal barrier damaged in the early phases of SAP. The COX-2-mediated inflammatory cytokines involved in this process.Selective COX-2 inhibitor(Celecoxib) selectively inhibit COX-2 expression in SAP.Celecoxib can significantly improve the permeability of intestinal mucosa and reduce the injury of intestinal mucosal barrier. Celecoxib is likely to prevent the systemic inflammatory response from continuous expanding. Selective cyclooxygenase-2(cox-2) inhibitor(celecoxib) might play a protective role in the intestinal mucosal barrier injury of severe acute panereatitis. |
