| Objective To explore the risk factors associated with fatty liver in patients with type 2 diabetes mellitus diagnosed firstly.Methods 139 patients with type 2 diabetes mellitus diagnosed firstly,were collected from the first affiliated hospital of ShanXi Medical University and make a retrospective analysis. Among these patients, 65 patients complicated with NAFLD and 74 patients did not . Record age and gender of patients, measure patients'body height,body weight,blood pressure ,make a bio-chemical examination of serum alanine transaminase(ALT),blood fat(TG,TC,HDL,LDL),fasting plasma glucose,fasting plasma C-peptite and insulin (FPG,FCP,FINS),postprandial 2h plasma glucose , 2h C-peptite and insulin(2hPG,2hCP,2hINS),renal function(BUN,GRE),24h urinary albumin(24hUALB),calculate body mass index , and evaluate insulin resistance with homeostasis mode . have a statistical analysis with SPSS13.0 software .Results (1) There were significant differences level of BMI,TG,HDL,FCP, 2h CP,FINS,ALT,HOMA– IR and 24 h UALB between with and without fatty liver in T2DM.(P <0.05,P <0.001) . (2) There were obvious increasing in the incidence rate in the obesity,lipid metabolic disorder,metabolic syndrome,diabetic nephropathy (P < 0.05 , P < 0.01) ).(3) Logisticregression analysis show that BMI,FCP and FINS is risk factor of with fatty liver in T2DM.(4) There were not significant differences level of age,gender,SBP,DBP,TC,LDL,2hPG,BUN,GRE between with and without fatty liver in T2DM. (P >0.05)Conclusion (1) There are obvious glucolipid metabolism abnormality and hyperinsulinemia in patients with NAFLD. (2) Obesity, insulin resistance and dyslip idemia are the chief risk factors of fatty liver in patients with type 2 diabetes mellitus. |
