Effect Of Hemocoagulase And Desmopressin On Coagulation Function Of Abdominal Operation Patients

Objective:Evaluating the function of coagulation of hemocoagulase and desmopressin using activated clotting time,prothrombin time and actived partial thromboplastin as index.Methods:Forty-five cases were randomly assigned to three groups. Their age is eighteen to sixty years old whatever the male or female.One group has been received hemocoagulase 2kU(Group H,n=15),the other has been received desmopressin(DDAVP)0.3μg/kg(Group D,n=15) infusion intravenously.The third group is control(Group C,n=15),the group has been received normal saline.There is no disease of haematic system and it's normal of platelet count,hemoglobin value,thrombin time(TT),prothrombin time(PT),actived partial thromboplastin(APTT), fibrinogen(Fbg).Not any case was received the remedy of anti-coagulation or intensify coagulation before operation.All patient's bleeding volume is less than 200ml.There is no transfusion blood during operation.The continual epidural anesthesia(CEA)was adopted.After entering the operation room,the patient's median cubital was punctured and 2ml venous blood was extracted for measuring ACT.The ACT was measured twice.The average value of two times is basic value.The basic value of PT and APTT has been measured before operation.Afterward,the venous blood was extracted in 5,15,30,60,90,120,150 and 180 minutes after using two drugs to measure ACT,PT,APTT.Results:1.Five minutes later,there is variance between the value of ACT and the basic value using hemocoagulase(0.01＜P＜0.05).In 10,15,30,60,90 minutes after using hemocoagulase,there is significance variance between the value of ACT and the basic value.It reached the peak in 10minutes(P＜0.01).The action of the drug become weakness in 120,150 minutes(0.01＜P＜0.05).There is no difference in 180 minutes(P＞0.05). 2.The action of desmopressin is similar to hemocoagulase.A little difference is that the effective appearing in 10 minutes after using desmopressin and reaching peak 30 minutes later.3.Comparing with basic value,the PT of H group and D group is not changed in different times.4.The APTT of H group is shortened in 5 minutes(P＜0.05)after using hemocoagulase and reach peak in 10 minutes(P＜0.01)and maintain 150 minutes(P＜0.05).There is no difference in 180 minutes(P＞0.05).5.The APTT of D group is shortened in 10 minutes(P＜0.05)after using DDAVP and reached peak in 30 and 60 minutes(P＜0.01).After 120,150 minutes,the APTT is prolonged(P＜0.05)and it's normal in 180 minutes(P＞0.05).Conclusions:1.The value of ACT was shortened in five minutes after using Hemocoagulase 2kU by intravenous injection.Ten minutes later,it's action reached peak and maintained 1.5 hours.Then,the efficiency of coagulation weakened.2.The value of ACT was shortened in ten minutes after using DDAVP 0.3μg/kg by intravenous injection and it's action reached peak in thirty minutes later and maintained 1 hour.3.Using activated clotting time(ACT)as index can test the function of coagulation of patients and the function of hemostasis.
