Objective To measure the line about the operative approach of pituitary adenoma by using dissected cadaver and clinical computer tomography, in order to provide anatomical prove for microsurgical operation.Methods To select adult formalin-fixed 30 cadaveric head specimens randomly which was filled by 10 percent red galatin and saw calvaria in ordinary anatomical method. To ectomize cerebral falx, calotte cerebral, ecotomize chiasm opticum, arteria carotis interna, cranial nerve third, intersect caudex cerebri and remain the structure of taking end-brain. To measure the distances of three different kinds of pituitary adenoma operative approach by sliding caliper .To observe the position relation of significance structure in appendices suprasphenoidalis. To rebuilding the base of 30 skulls by CT, measure the distances of arteria carotis interna eminence and canalis opticus eminence in external sinus sphenoidalis. The angle and depth is from anterior wall of sinus sphenoidalis to nares to analysis the all dates by SPSS11.5.Results Performed specimens of cranial base and measured the distances between those bong marks successfully. Accomplished craniotomy observation and measurement of the important neurovessel. There were statistical differences(P<0.05) between the length of 3 lines of transfrontal approach (distance from Midpoint of superciliary arch superior border to the homonymy entrance of ICA, distance from Midpoint of superciliary arch superior border to the opposite side entrance of ICA, distance from Midpoint of superciliary arch superior border to the homonymy lap of ICA) and 3 lines of pterional approach in group CT and homologous lines in group specimen; there were not statistical differences(P > 0.05)between two groups in all lines of transsphenoidal approach and other 4 lines of transfrontal approach.Conclusion It is very necessary to combine applied anatomy with topographical anatomy to study and master the anatomy and variance of this approach. The first choose to accomplish the pituitary adenoma surgery was passing through the sphenoidal sinus by pre-surgery clinical tomography.
