Objects:The purpose of the present study was to study the effects of retinal detachment surgeries on retrobulbar haemodynamics in patients with rhegmatogenous retinal detachment(RRD).Methods:1.Patients:Color Doppler flow imaging(CDFI)was used for measurement of blood flow velocities and resistive indexes of the ophthalmic artery(OA),posterior ciliary arteries(PCA)and central retinal artery(CRA)in 96 eyes of 96 patients with RRD. CDFI parameters were obtained before and 3 months after surgeries.The type of surgeries was chosen according to the intraocular conditions.Scleral buckling surgeries(SBS)were conducted in 40 eyes,and vitreoretinal surgeries(VRS)were done in 56 eyes combined with usage of PFCL,photocoagulation and silicone oil injection.The patients had been followed for 2-4 months.2.Color Doppler Flow Imaging:Preoperative and 3 months postoperative CDFI examinations were performed on peak systolic velocity(PSV),end diastolic velocity (EDV),resistance index(RI)detections of CRA,shot posterior ciliary artery sPCA and Ophthalmic artery OA.The fellow heath eye was set as control.3.Groups:Preoperative:●RD eye compared with control eye preoperatively,including:■SBS group:RD eye compared with control eye.(n=40)■VRS group:RD eye compared with control eye.(n=56)●Preoperative comparison between groups of SBS and VRSPostoperative:●SBS group:RD eye comparison between post- and pre- operation,and RD eye compared with control eye postoperatively.(n=40)●VRS group:RD eye comparison between post- and pre- operation,and RD eye compared with control eye postoperatively.(n=56)●Postoperative comparison between groups of SBS and VRS4.Statistical Managment: All data were normally distributed and represent arithmetic mean±SD.Student's t-tests were used to compare the data by SPSS11.3.P<0.05 was considered significant.Results:1.PSV,EDV,RI of CRA,sPCA and OA in RD eyes have no statistical different compared with control eyes(P>0.05).SBS group has no different parameter with VRS group(P>0.05).2.In SBS group:PSV and EVD of CRA,sPCA were significantly decreased after surgeries,RI were increased significantly(P<0.05).Parameters of OA have no change after surgeries(P>0.05),but also no change was found in every parameter compared with control eye.3.In VRS group:PSV and EVD of CRA,sPCA were significantly increased after surgeries,RI were decreased significantly(P<0.05).Parameters of sPCA and OA have no change after surgeries(P>0.05),but also after surgery,no change was found in every parameter compared with control eye.4.Comparison between SBS and VRS groups:PSV and EVD ofCRA,sPCA,and OA were significantly lower in SBS group(P<0.05),while RI was significantly higher(P<0.05).Conclusions:1.CDFI parameters of CRA,sPCA and OA in RD eyes have no change compared with control eyes.2.SBS might decrease surgery eye blood flow by increasing RI of ocular vessels while decrease velocities of CRA and sPCA,which might belong to mechanical stress of extraocular scleral materials.3.VRS might increase the velocity of CRA,decrease RI and improve ocular blood supply postoperatively.
