Background:Recent epidemiology researches have demonstrated that systolic pressure and pulse pressure are better in predicting the cardiovascular disease comparing with diastolic pressure.The increased stiffness of large artery is a dominant factor of systolic and pulse pressure increasement.Pulse wave velocity,as an index evaluating the stiffness of large artery,has raised vast attention and deep research,and is also recognized as an important factor evaluating the risk and prognosis of hypertention and coronary artery disease.Objective:To identify the correlation of arterial stiffness and the difference of systolic pressure of brachial artery using auscultatory method and palpation method,and to find a feasible easy shortcut in evaluating the elasticity of large artery.Method:The systolic pressure of the right arm were measured in the morning and then calculated their difference(ΔP).The brachial-ankle Pulse Wave Velocity(baPWV) were also measured as the parameter responding to the arterial elasticity,using the automatic measuring instrument VP-1000 made in Japan.Fifty-one patients with cardiovascular disease were included(man 40,woman 11,age between 37-78,mean age 56.43±10.22).All the data was analyzed with SPSS13.0 statistical software.P<0.05 was considered statistically significant. Estimate the best cutoff point by receiver operating characteristic curve to predict the arterial stiffness.Results:(1)The linear regression analysis showed that PWV was closely related to age(r=0.614,p<0.01) and systolic pressure measured in auscultatory(r=0.314,p<0.05) and palpation(r=0.443,p<0.01) method.(2)The linear regression analysis showedΔP and baPWV was also closely correlated (r=-0.476,p<0.05).(3)A receiver operating characteristic curve demonstrated that the best cutoff point of the difference of systolic pressure of brachial artery using auscultatory method and palpation method for predicting the arterial stiffness was 7mmHg.Conclusion:The arterial elasticity and the difference of systolic pressure of brachial artery by auscultatory palpation methods was negatively correlated.The arterial elasticity goes large with the difference diminishing. Clinical doctors can predict the stiffness of artery by the difference of systolic pressure of brachial artery using auscultatory and palpation method.
