Objective: The distribution of tinea pedis is worldwide. In professions on hermetic rubber shoes, the prevalence of tinea pedis arrives higher than 80%. The pathomycete is most of Trichophyton rubrum. The unstandard treatment induces the recurrence of tinea pedis and the production of drug-resistance fungus species. The studys in the near future, indicate that innate immunity plays critical roles in antifungal immunity, to kill the pathogen directly, furthermore to stimulate the adaptive immunity. Identification the invaded fungus is the first step of stimulating the host produce immune response. Fungus infection stimulates phagocytic cells activation, which excrete a lot of cytokines. IL-12 is one of the crital factors which stimulatis innate immunity and activates Th1 cell to mediat specific immune response. IL-12 is especially important because it regulates innate immunity and determines the type and duration of adaptive immunity. IL-12 induces IFN-γand other cytokines production by NK, T cell , dendritic cells, macrophages and Th1 cells. IFN-γis a potential factor which can induce IL-12. Its production depends on IL-12, theremore IFN-γproduction increase secretion of IL-12 in turn. IFN-γaffect macrophages to produce iNOS. Then it produces NO, which is the important factor for clearing fungus pathogen. NO plays an important role in adjusting the inflammation reactive, which has toxic action to etiological agent. These four kinds of cytokine construct a positive feedback path. As a functional unit, they can enhance specific immune response and become the key position between innate and adaptive immune response. In this study, we detect IL-12,IFN-γand iNOS,NO in the serum level of tinea pedis patients from whom Trichophyton rubrum has been isolated. The purpose of the study is to detect the serum level of these four kinds of cytokine in tinea pedis patients and discuss their function in the immunologic mechanism of tinea pedis.Methods: 1.case seclecting: The case group involved thirty tinea pedis patients in outpatient clinic in the second hospital of Hebei Medical University. Including standards: All the patients were tinea pedis patients. Test under microscope was positive and Trichophyton rubrum had been cultured. The age of all the patients in study were between 18 and 70, women in which were not pregnant and lactation. All the cases involved had not taken antimycotic drug for one week, or taken antifungal agent for one month, or drugs that could affect immunological function for two months such as corticosteroids and immunosuppressive agent, et al. Excluding standards: patients with autoimmunity diseases such as erythema lupus, pemphigus, dermatomyositis, rheumatic arthritis, et al, and patients with tumor, with other severe Systemic diseases. The control group involved twenty healthy people without tinea pedis, autoimmunity diseases and family history. There were no significant deviation in the age and sex between case group and control group by t test. 2.Gaining the patients'permission , their blood (5ml) were collected and separated and stored at -20℃. 3. material collecting and fungus clturing: To scrape scale or shear the bleb wall with blunt lance in pathological changes, if test under microscope was positive, fungus culture was done at the same time. We observe the cultural condition intimatly under constant temperature 25℃. It was thought to be negative assessment, when no colony grows for four weeks. 4. strain identification: according to the colonial morphology and the cultural construction under microscope, Dermatophyte could be definited the species. 5. cytokine detecting: Serum levels of IL-12 and IFN-γwere detected by ELISA according to directions in the test kit. Serum levels of iNOS and NO were detected by 721 spectro photometer according to directions in the test kit. 6. Data analysis: All values were expressed as mean±SD compared with controls and among separate experiments. Independent-sample T tests were used to determine the significance of changes in serum concentrations of these four kinds of cytokines. P values of less than 0.05 were considered significant.Results:1. IL-12 serum concentrations were lower in tinea pedis patients than in controls. There was significant deviation between case group and control group(P<0.05). 2. IFN-γserum concentrations were lower in tinea pedis patients than in controls There is significant deviation between case group and control group (P<0.05).3 The serum concentrations of iNOS were lower in tinea pedis patients than in controls There is significant deviation between case group and control group(P<0.05).4.There was lower expression of NO in tinea pedis patients than in controls There is significant deviation between case group and control group (P<0.05).Conclusion:1. The serum level of IL-12, IFN-γ, iNOS, and NO in tinea pedia patients from whom Trichophyton rubrum has been isolated is lower.2.The immune function of in tinea pedia patients from whom Trichophyton rubrum has been isolated is lower. Maybe chronic infection of Trichophyton rubrum in tinea pedis patients relate to the lower expression of these four kinds of cytokines .
