| Objective: Diarrhea is one of the most frequent diseases in the childhood and splenasthenic diarrhea accounts for a large of proportion of child diarrhea. It is likely to result in malnutrition, rickets, anaemia and hypoimmunity just because it often has the long course of disease, recurs and is difficult to recover, thus it greatly affects the health and growth of the children. Modern medicine frequently adopts maintaining the balance of Water-Electrolyte and acid and alkali, supporting treatment. There is still no special therapeutic measure to deal with it, and thus failing to solve fundamental question. But the therapeutic effect on this kind of disease is satisfactory through using traditional Chinese medicine just because it has many priorities which includes overall efficacy, little side effect and so on.After many years'clinical researches, my tutor considered that the mechanism of splenasthenic diarrhea was related to the weakness of spleen and stomach, which fails to receive food and transport and transform refined nutritious substances, and thus contributes to food retention and the combination of clear QI being mixed up with turbid QI. And he study out many methods combining such as nourishing qi and invigorating spleen, activating spleen-energy and promoting digestion, damp elimination to check diarrhea. Under the guidance of the way and the combining the modern studies of the herb pharmacological, we make the reasonable combined prescription of Chinese herb-YiPiZhiXie decoction (YPZXD), which is composed of Atractylodes macrocephala, Atractylodes chinensis, Tuckahoo, Heterophylly falsestarwort root, Jineijin, Charred FRUCTUS CRATAEGI, Herba Agastachis, Lianzirou, Semen, Cassia, costustroot and so on. It has been used for more than twenty years in clinic and has the dependable curative effect. In order to investigate the therapeutic effect and the mechanism of YiPiZhiXie decoction (YPZXD) on child splenasthenic diarrhea and provide theoretical foundation for the clinical application of it , we choose splenasthenic diarrhea model of pedo-rats induced by Rhei Decoction and take smecta as positive control drug through observing the change of general state of health, the content of D-xylose in blood serum and urine, the content of AMS in urine, the spleen and thymus coefficient, the content of MTL in blood plasma, the content of SDH in blood serum.Methods: The making model process followed the method (bitter cold decantation) used in the literature. Adapting surroundings for seven days in the laboratory before experiment, the 60 SD pedo-rats were divided into five groups in random and average, which included the normal group, the model control group ( the model group for short ), the smecta control group (the smecta group for short), the small-dose of YPZXD (the small-dose group for short) and the large-dose of YPZXD (the large-dose group for short), and there are 12 rats in each group. After weighing all rats on the first day, each rat in the normal group was drenched with 2ml/100g of NS while the rats in other groups were drenched with 2ml/100g of 100% Rhei Decoction (1g·gruffs /ml) twice a day and this would contine for nine days. In the course of making model, all rats freely take a meal and drinking. After weighing all rats on the tenth day, we infused 1ml/100g of D-xylose solution with the concentration of 3% to all rats and the blood samples would be drawn from the tails of the rats one hour later in order to measure the content of D-xylose in the serum. Then we begin to intragastric administration and infuse the 2ml/100g of NS to the normal group and model group, the 2ml/100g of smecta solution with the concentration of 40mg/ml to the smecta group (equivalently 10 times clinic isoeffect dosage), the 2ml/100g of YPZXD with the concentration of 1.65g/ml to the small-dose group (equivalently 10 times clinic isoeffect dosage) , the 2ml/100g of YPZXD with the concentration of 3.3g/ml to the large-dose group (equivalently 20 times clinic isoeffect dosage) once a day which would continue for a week. In the course of treatment, all rats in each group freely take a meal and drinking. All rats were fasted at 10:00 on the night of the 16th day. After weighing all rats at 5:00 on the morning of the 17the day, we infused 1ml/100g of D-xylose solution with the concentration of 3% to all rats and put them the metabolism cages so as to collect the rats'urine for 5 hours so as to measure the contents of D-xylose and AMS in urine. A hour later, the blood samples would be drawn from the tails of the rats in order to measure the content of D-xylose in the serum. At ten on the morning of the 17th day, all rats were put to death and the blood samples were collected to provide the blood plasma and blood serum for measuring MTL in the blood plasma and SDH in the blood serum. Moreover, the spleen and thymus were taken out from the abdominal cavity and cleaned and weighed in order to calculate the organ coefficient.Results:1 The effect on body weight to splenasthenic diarrhea pedo-ratsOn the 1st day (before making model), there were not significant difference among the five groups on body weight (P>0.05); On the 10th day (before treating), there were significant difference between normal group and the rest group (P<0.01) while there was not significant difference among the experimental groups(P>0.05). On the 17th day, there were significant difference between the high-dose group, the low-dose group, the smecta group and the model group (P<0.05, P<0.01); at the same time, the comparison between the high-dose group, the low-dose group, the smecta group and the normal group had significance (P<0.05, P<0.01); there were significant difference between high-dose group, low-dose group and smecta group (P<0.05), this explains that both two kinds of medicine can promote taking in nutrient substance so as to increase in the body weight of splenic asthenia pedo-rats and that the effect of YPZXD is better than that of smecta.2 The change of the immune organ coefficientSpleen coefficient: The difference was significant when the every experimental group was compared with the normal group (P<0.001); there were not significant difference between the small-dose group and the model group (P>0.05); Compared with the model group and the small-dose group, the large-dose group and the smecta group reduced in the spleen coefficient, and the difference was significant (P<0.01), however, there was no statistical significance between the large-dose group and the smecta group (P>0.05). This explains that the immune function of rats reduced after making model and that both YPZXD and smecta can increase in the immune function. Thymus coefficient: Except the model group, the thymus coefficient of the rest groups had no change compared with the normal group, and there was no statistical significance (P>0.05). This explains that splenasthenic diarrhea pedo-rats have T lymphocytic abnormality and that both YPZXD and smecta can protect T lymphocyte.3 The effect on the content of D-xylose in blood serum of splenasthenic diarrhea pedo-ratsAfter making model, compared with the normal group, the every experimental group decreased in the content of D-xylose in blood serum and there were significant difference (p<0.01); But there was not significant difference among all experimental groups (p>0.05), which suggested that making model is successful. After treatment, the every experimental group increased in the content of D-xylose in blood serum. There were significant difference between the model group and the each treatment groups (P<0.05, P<0.01); In addition, the difference between the smecta group, the small-dose group and the normal group has statistical significance (P<0.05) while the large-dose group approached the content of D-xylose in blood serum as the normal group. This indicates that the large-dose of YPZXD can increase the content of D-xylose in blood serum to the normal level.4 The effect on the contents of D-xylose and AMS in urinethe content of D-xylose in urine: There were significant difference between the large-dose group, the smetca group, the normal group and the model group (P<0.001); The comparison between the large-dose group, the smecta group and the normal group had no significance (P>0.05). This means that both the large-dose YPZXD and smecta have the good function of increasing in the content of D-xylose and thus enhance the intestinal absorption function of splenasthenic diarrhea rats. The content of AMS in urine: There were significant difference between the model group and the every treatment group (P<0.001); There was no significant difference between the large-dose group, the smecta group and the normal group (P>0.05), which showed that both the YPZXD and smecta have raise AMS activity and thus enhance the function of pancreatic external secretion.5 The effect on the content MTL in blood plasmaMTL in blood plasma greatly increased in the model group and the difference between the model group and the normal group (P<0.001). There were significant difference between the large-dose group, the small-dose group, the smecta group and the model group (P<0.01). The comparision between the large-dose group and the smecta group has statistical significance (P<0.05), which showed that YPZXD can retrieve the function of indiscriminate stomach intestine by regulating the level of MTL in blood plasma.6 The effect on the content of SDH in blood serumCompared with the normal group, the model group, the smecta group and the small-dose group are decreased significantly in the content of SDH in blood serum (P<0.01, P<0.001). On the other hand, the content of SDH increased in the large-dose group and there is no significant difference between the large-dose group and the normal group (P>0.05). And there was statistical significance (P<0.05) between the large-dose group and the smecta group. The result indicates that the large-dose YPZXD is more able to enhance aerobic metabolic function of splenasthenic diarrhea rats'cell than the smecta.Conclusion: YiPiZhiXie Decoction can evidently check diarrhea and encourage splenasthenic diarrhea pedo-rats to take in food and accrescence their body weight, it may produce a maked effect by increasing in D-xylose and AMS activity and thus regulate the intestinal absorption function and enhance the function of pancreatic external secretion. In addition, it may contribute to the spleen functional recovery. On the other hand, it may retrieve the function of indiscriminate stomach intestine by regulating the level of MTL in blood plasma and increasing in the content of SDH in blood serum. |
