ObjectiveTo investigate the living state of Patients with chronic liver disease, to analysis the available associated factors.Methods347patients who had chronic liver disease (include Chronic Hepatitis B,C , Alcoholic liver disease and Non-alcoholic fatty liver)were selected and followed up for 2-7 years ,Preparation pathological grade, Child -Turcotte-Pugh grade, size of the liver, spleen size, portal width, age and other variables influencing factors for survival in patients with chronic liver disease. Using a single factor analysis and Cox regression analysis determined under the conditions of multiple factors affect the survival of patients with factors. According to Child-Turcotte-Pugh classification and the biopsy results were divided into three groups, analysis each group of survival and the survival curve. Patients in all 347 cases were followed up at the end of the survival of state variables, confirmed the death to the survival time For the time variable, with follow-up survey of 12 related factors as variables, single-factor analysis and Cox regression analysis on the factors affecting patient survival.Results( 1) There were 347 patients, a ratio of 4.6:1 men and women, aged 14-64 years old, the average age of 36.9±10.0 years, patients with chronic hepatitis has been accounted for 79.8 percent. an average of 48.4 months follow-up time, a total of 12 patients died, the average mortality rate was 3.45%。( 2) According to Child-Turcotte-Pugh classification (A, B, C-class) the median survival time was 88 months, 85 months, 29.6 months, C-class patients with the A, B-class compared them with median survival time was shorter Obviously.( 3) According to biopsy results into mild chronic hepatitis (G1-2, S0-2), moderate chronic hepatitis (G3, S2-3), severe chronic hepatitis (G4, S3-4), which median survival time were 88 Months, 88 months, 72 months, severe chronic hepatitis which median survival time was significantly shorter than in the previous two groups.( 4)The whole group 347 cases were divided into groups and the survival of the death group, will follow-up survey at the 12 factors related to single-factor analysis showed that age, the result of biopsy, bilirubin, albumin, PTA, whether complications in the two groups there were significant differences (P <0.000-0.005).(5) Multi-factor analysis showed that survival of patients with chronic liver disease as the main factors: the results of the biopsy, age, bilirubin and prothrombin activity (P <0.000-0.05).(6) The prognosis for the model: R = 1.841×pathological grade +0.13×age +0.012×serum total bilirubin (μmol / L) -0.043×prothrombin activity.Conclusions(1)This study showed that pathological grade, age, bilirubin, albumin and prothrombin activity and whether complications are major factors that affect the survival rate of patients with chronic liver disease. The pathological grade, age, bilirubin, prothrombin activity are key factors affecting the survival rate of patients with chronic liver disease.(2)According to the prognosis model, the study suggested that the seizure of results at the greatest degree of value, that is its greatest impact on the prognosis, followed by the impact of age, while minimizing the impact of bilirubin. Prothrombin activity coefficient is negative, that negative correlation with the prognosis.(3)Male patients with chronic liver disease significantly more than women, but gender in survival between the group and the death group no significant difference. The incidence of chronic liver disease to most middle-aged and young, the elderly patients with relatively poor prognosis(4)This study once again prompted to chronic liver disease in China mainly to chronic hepatitis B,and suggested that early intervention on clinical liver disease process, the delay may be extended or improved liver function in patients with the survival time, reduce mortality.
