The Multiple Regression Analysis Of The Correlation Between Early Measurement Of Arteriosclerosis And Occurrence Or Risk Factors Of Cardiovascular Disease

Backgroud:The typical pathological change of vascular structure and it's function is arteriosclerosis and atherosclerosis,that is the pathologic base of the cardiovascular and cerebrovascular disease such as myocardial infarction and brain stroke. Brachial-ankle pulse wave velocity(baPWV)and ankle- brachial index(ABI)are the indexes to evaluate the vascular structure and it's function.Whether they correlate with the risk factors of cardiovascular is not clear,and whether they can be quantitative predictors for the cardiovascular disease is not clear yet.Subjects:1,To make clear the relationship between baPWV and ABI as the indexes to evaluate the arteriosclerosis and occurrence or risk factors of cardiovascular,and to find out their clinic values.2,Try to use ABI and baPWV as indictors of atherosclerotic heart disease-coronary heart disease(CHD).3,To study the correlation between arterial elasticity and atherosclerosis.4,To study the effects of nitroglycerin on value of ABI and baPWV.Methods:We recruited 1000 individuals(713 male,287 female,69.51±15.18 average years old)including inpatients or outpatients in people's hospital of JiangXi Province.ABI and baPWV were measured by the VP-1000 automatic arteriosclerosis measurement system.Coronary angiograph was performed in 150 patents.Doppler echocardiographic examination for carotid artery was performed in 154 patients.Results:1.The relationship between ABI and occurrence or risk factors of cardiovascular disease:ABI＜0.90 related with smoking(OR=2.245,95%CI=0.854-5.421),hypertension (OR= 1.736,95%=0.556-4.371)and CHD(OR=2.321,95%CI=0.486-4.281)(P＜0.05), while ABI＜0.9 not relate with age,body mass index(BMI),hypedipidemia(HLD), ischemic stoke and diabetes(P＞0.05).2,The relationship between baPWV and occurrence or risk factors of cardiovascular disease:After age adjusted,arteriosclerosis evaluated by baPWV related with hypertension directly(OR=6.528,95%CI=3.137-12.763),smoking(OR=6.446,95%CI =2.411-17.235),hyperlipidemia(OR=4.694,95%=2.200-10.014),gender(male) (OR=3.100,95%CI=1.106-8.687)and CHD(OR=1.795,95%CI=1.020- 3.894)were also the related factors of arteriosclerosis(P＜0.01).The incidence of arteriosclerosis is increasing as the number of risk factors of cardiovascular disease adding.3,The value of ABI and baPWV as indictor of CHD:It was statistic significant difference in the value of ABI and baPWV between the positive and the negative group of coronary angiograph.It was also found that baPWV＞1700cm/s as indictor of CHD was more valuable than ABI＜0.91 in the sensitivity(83.33%vs 30.70%),veracity(84.00%vs 44.67%)and value of Kappa (0.61 vs 0.11).4,The relationship between carotid atherosclerosis(CAS)and baPWV:CAS related with age,baPWV,smoking and pusle press(P＜0.05),but didn't relate with hypertension(P＞0.05).As baPWV increasing,the OR of carotid atherosclerosis was raising.With age and baPWV adding,the morbidity of CAS was increasing.5,the effects ofnitroglocerin on the baPWV and ABI:After taking medicine nitroglocerin,baPWV in all persons diminished. (P＜0.01).The change of baPWV was positive correlation with systolic pressure(SBP) before taking medicine and age.The value of ABI increased after taking medicine(P＜0.01).The change of ABI was negative correlation with the heart rate(HR)before taking medicine,and was positive correlation with HR after taking medicine.Conclusion:1.ABI＜0.90 related with smoking,hypertension and CHD.Arteriosclerosis evaluated by baPWV related with hypertension,smoking,hyperlipidemia and gender.As the risk factors of the cardiovascular disease added,the incidence of arteriosclerosis increased.ABI and baPWV could become unaided predictors of cardiovascular disease.2.baPWV could be indictor of CHD.3.baPWV could be a predictor of carotid atherosclerosis.4.Nitroglocerin could improve baPWV and ABI and their clinic value should be further to research.
