Objective To study the diagnosis, differential diagnosis and surgical methods of salvage therapy for acute lower extremity ischemia(ALEI).Methods A retrospectively analysis was made on the clinical data of 102 patients with total 121 ALEI limbs who were admitted to our department from December 2001 to December 2007.When the states of the illness were clear, Fogarty catheter embolectomy was used as the initial treatment for ALEI in a regular operating room or in an intervention therapy room under DSA monitoring on 82 cases(99 limbs), and 72 patients(74 limbs) receive transcatheter intraarterial infusion of thrombolysis drugs after embolectomy, also another patient receive the "kissing stent" after the embolectomy. endarterectomy after embolectomy was performed on 9 cases(11 limbs), other 11 patients(11 limbs) were treated with autosaphenous vein bypass.Results the salvage of 90 cases (106 limbs) was successful, of which 16 patients (18 limbs)had sensory and motor sequela in different degrees. 8 cases had amputation .4 cases died of acute myocardial infarction or acute renal failure.Conclusion The acute lower extremity ischemia is a condition of several diseases,we must be aware of the differential diagnosis .Meanwhile we must grasp the indication and contraindication of the therapy,and choose the best method. At the same time we must particularly pay attention to the prevention of the complication of acute lower extremity ischemia.
