Objective: To find out the distribution characteristic and regularity of pulse pressure of people through an epidemiologic survey and direct people to undertake pulse pressure control. To analyze the association of BMI,WHR and pulse pressure and direct people to establish a healthy lifestyle,control body weight,maintain normal somatotype and avoid the risk of cardiovascular disease.Method: The objects are the health examination datum of 6427 civil servants of Shijiazhuang who were examined in the medical examination center of the second hospital affiliated to Hebei Medical University from April 2006 to July 2006. There are 3505 men whose mean age is 50.60±13.87 years and 2922 women whose mean age is 48.04±14.07 year. To group all the objects according to different gender and age stage and calculate the pulse pressure of each group ( x±s), and the comparison method between groups is T test. To divide the pulse pressure to 4 stages which are <52mmHg,52~68 mmHg,69~85 mmHg and≥86 mmHg according to the mean and standard deviation of pulse pressure and statistics the distribution frequency of each stage among all the objects. To define PP≥60 mmHg as broad pulse pressure and compare the incidence rate of broad pulse pressure among youth(20~39years),middle-aged(40~59years) and senium(60~91years). The method applied is chi-square test. Using Ministry of Health, Disease Control Department 2003 Chinese adult guideline on overweight/obesity prevention and control the total cases were divided into 3 groups according to their body mass index(BMI), which are BMI<24.0,BMI24.0~ and BMI28.0~. The incidence rate of the overweight and the obesity of the civil servants are calculated and the comparison method among different groups is chi-square test. Meanwhile , the pulse pressure of different age and different BMI is also calculated . The comparison method between obesity and normal body weight is T test. Among all the examined civil servants there were 3878 cases whose waistline (W) and amb-hip (H) had been measured. WHR=W/H. Based on the medical handbook edited by Gong Lansheng in August 2005, WHR>0.90 (male) and WHR>0.85(female) is defined as abdomen-type obesity. To calculate the pulse pressure of abdomen-type obesity people and non- abdomen-type obesity people of different age groups. The comparison method of the pulse pressure of abdomen-type obesity and non- abdomen-type obesity in each group is also T test. All the data is put into computer through Excel2003 and is analyzed statistically by SPSS13.0. P<0.05 is regarded as having statistical significance.Result: There are 6427 civil servants who accepted medical examination, and their mean age is 49.43±14.02years, mean systolic blood pressure (SBP) is 130.78±21.05mmHg, mean diastolic blood pressure (DBP) is 78.97±12.64mmHg, and mean pulse pressure (PP) is 51.80±15.75mmHg. There are 3505 men whose mean age is 50.60±13.87years, mean SBP is 135.03±20.31mmHg, mean DBP is 82.37±12.20mmHg, and mean PP is 52.65±14.97mmHg. There are 2922 women whose mean age is 48.04±14.07years, mean SBP is 125.68±22.53mmHg, mean DBP is 74.90±11.93mmHg, and mean PP is 50.79±16.58mmHg. Among all the civil servants the percent of the young (20~39years) is 28.29%, the percent of the middle-aged is 43.33% and the percent of the senium is 28.38%. The result of one-way ANOVA indicates the pulse pressure of male civil servants is increasing with age after 40 years old and the pulse pressure of female civil servants is increasing with age after 30 years old. T test result of the same age group between male and female indicates the pulse pressure of male civil servants is higher than that of female in 20~29years,30~39years and 40~49years(t=7.44,12.03,3.67,P<0.001)groups and the pulse pressure of female civil servants is higher than that of male in 50~59years,60~79years and 80~91years groups. The highest pulse pressure distribution frequency is at <52mmHg and the percent is 58.47%. The higher the pulse pressure, the smaller the proportion of the people. The highest distribution frequency of pulse pressure of the young and the middle-aged is 79.81% and 69.91% (at the <52mmHg level). The highest distribution frequency of pulse pressure of the senium is 40.63% (at the 52~68mmHg level). No matter male or female the proportion of PP≥60mmHg is higher in the people whose age is≥60years than that of the <60years people. The overweight percent and obesity percent of the civil servants examined is 39.10% and 14.67%. In general, the overweight percent and the obesity percent are increasing with the age of the objects. The overweight and obesity circumstance of the civil servants is serious. The pulse pressure of the obesity is higher than that of the normal body weight (P<0.05). For male, the pulse pressure of the young and middle-aged abdomen-typed obesity is higher than that of the non-abdomen-typed obesity (P>0.05), and the pulse pressure of the old abdomen-typed is higher than that of the non-abdomen-typed obesity (P<0.001). For the female the pulse pressure of the abdomen-typed is higher than that of the non-abdomen-typed (P<0.05). All in all, the pulse pressure of the obesity is higher.Conclusion: The pulse pressure of the civil servants of Shijiazhuang shows the tendency of increasing with age and the tendency is more notable. Before the age of 50 years the pulse pressure of male civil servants is higher than that of female and after the age of 50 years the pulse pressure of female civil servants is higher than that of male. The reason probably is the compliance of vessel wall of young women is better than that of man, and with the estrogen level descending post-menopause the rigidity of the vessel wall accelerating, the difference between men and women has disappeared. The middle-aged and the old especially the old is more likely to have broad pulse pressure. Pulse pressure is one of the independent risk factors of cardiovascular disease. The cardiovascular disease is more likely to happen to the middle-aged and the old. So the middle-aged and the old people should control pulse pressure and measure blood pressure regularly. When the increased pulse pressure is discovered, timely and reasonable medical measures should be adopted. The matter of overweight and obesity among the civil servant is serious and their rates are increasing with age. It is known that obesity can result in a series of diseases. The civil servants should think highly of this problem and take appropriate steps to decrease body weight. In this study, the pulse pressure of the obesity is higher. Probably there is some association between obesity and higher pulse pressure.Recently many investigations have indicated that the pulse pressure is one of the independent risk factors of cardiovascular events. As is known obesity is one of the risk factors of cardiovascular disease. Therefore, lowering body weight,decreasing fat accumulation and controlling pulse pressure reasonably could contribute to avoiding the risk of cardiovascular disease.
