| Objective: To investigate Midkine(MK) levels in serum and urine of patients with gastric carcinomas before and after operation, detect the expressions of MK in gastric carcinomas tissues, study their relations to the biological characteristics of gastric carcinomas, clarify the role of MK in the invasion and metastasis of gastric carcinomas. At the same time,provide evidence to disclose the malignant biological behavior of gastric carcinomas,and get an objective evaluation and judgment of diagnosis,curative effect and prognosis for patients with gastric carcinomas.Methods: The levels of serum and urine MK were detected by using enzyme-linked immunosorbent assay(ELISA) in 20 healthy cases and 45 patients with gastric carcinomas when the day before operation and the seventh-day after operation. the expression of the MK protein were detected using immunohistochemical stain in specimens of 45 surgically removed human gastric carcinomas .Results:1.The average optical density(O.D.) of MK in serum of gastric carcinomas patients before operation was 0.496±0.149,0.363±0.129 in after operation group,0.292±0.085 in control group. The optical density of MK in serum of gastric carcinomas patients before operation were significantly higher than those after operation and those in healthy controls(P<0.001).2.The average optical density of MK in urine of gastric carcinomas patients before operation was 0.091±0.051,0.049±0.036 in after operation group, 0.029±0.019 in control group. The optical density of MK in urine of gastric carcinomas patients before operation were significantly higher than those after operation and those in healthy controls(P<0.001).3.The serum level of MK in before operation group was not related to age,gender,differentiation , pathologic type and tumor size in the patients with gastric carcinomas(P>0.05),but significantly related to TNM stage and lymph node metastasis(P<0.05). 4.MK positive expression was 77.8% in gastric carcinomas tissues, no expression in normal gastric tissues. The MK expression in gastric carcinomas tissues was significantly higher than that in control group(P<0.001).5.The MK expressions in gastric carcinomas tissues were not related to gender,age,differentiation,pathologic type and tumor size(P>0.05), Meanwhile there were significantly statistic difference between TNM stage and lymph node metastasis (P<0.05).Conclusions:1. Overexpression of MK in gastric carcinomas tissues, blood and urine were 1inked with the occurrence of gastric carcinomas.2. The optical density(O.D.) of MK in serum and urine of gastric carcinomas patients before operation were significantly higher than those after operation , This indicates that the high level of serum and urine MK in our case subjects may be the result rather than the cause of their gastric cancer, which may serve as an evaluation for recurrence after operation.3.The MK expressions were not related to gender, age, differentiation and pathologic type and tumor size, Meanwhile there were significantly statistic difference between TNM stage and lymph node metastasis, it suggest that the progression stage of cancer rather than the histological type or differentiation of cancer affects S-MK concentrations more strongly. Increased levels of serum MK with tumor progression may promote tumor invasiveness ,its mechanism needs further study.4.The combined detection of the MK expressions in tumor tissues,blood and urine may be more useful to earlier diagnosis,tumor progress, judging prognosis and follow-up of patients with gastric carcinomas. |
