Clinical Observation On Chronic Renal Failure And Experiment Study On Adenine-Induced Chronic Renal Failure Model In Rats With Shenqi Jiedu Decoction

Objectives:Glomerular sclerosis and interstitial fibrosis are the most important patho fundaments.As there is not any special treatment to chronic renal failure,we observed the effects of Shenqi Jiedu decoction on chronic renal failure of deficiency of both qi and yin and turbid and blood stasis syndrom and on rats of Adenine-induced chronic renal failure.Our purpose is to approach the effects of the prevention and the mechanism of Shenqi Jiedu decoction.Methods:(1)Clinical study:60 CRF cases of deficiency of both qi and yin and turbid and blood stasis syndrom were randomly divided into therapy group(treated with Shenqi Jiedu Decoction,general western medicine and losartan)(n=30)and controlled group(using general western medicine and losartan)(n=30).Each treatment period was 4 weeks and there were two periods.The changes of total effective rate,serum creatinine(SCr),blood urea nitrogen(BUN),creatinine clearance rate(Ccr),hemoglobin(Hb),plasma albumin(ALB),triglyceride(TG),cholesterol(CHO) and adhesion molecule in two groups were compared after treatment.(2)Animal experimental study:Adenine-induced chronic renal failure model in rats was established.All animals were randomly divided into five groups:the normal group, the model group,the high dose group,the low dose group and losartan group,After 8 weeks of treatment,the levels of SCr,BUN,TG,CHO,ALB,SOD were tested in the five groups.In addition,the results of pathology and the expressions of TGF-β₁ and FN were observed.Results:(1)Clinical study:The total effective rate of the treating group was 86.67 percent,while the control group was 60.00 percent,The comparison of the curative effect in two groups was significant(P＜0.05).In two groups SCr,BUN were both decreased,and Ccr,Hb,ALB were all increased.The decrease of SCr,BUN,and increase of Ccr,Hb in the treated group was more significant than in the control group (P＜0.05).Meanwhile in the two groups the expression of ICAM-1 and VCAM-1 in peripheral blood were decreased,and the increase of the treated group was more significant than that in the control group.(2)Experimental study:Compared with the model group,the levels of SCr and BUN in Shenqi Jiedu Decoction groups and losartan group were lower,while the levels of ALB were higher(P＜0.05).The contents of SOD in blood-serum were increased in The high dose group,the low dose group and the losartan group.And the contents of SOD were higher in the high dose group and the low dose group than in the losartan group.And pathology also proved that the results of Shenqi Jiedu Decoction groups and losartan group were better than the model group,and the high dose group was the best.The gray scale of the immunohistochemistry staining indicated that the expression of TGF-β₁ and FN were improved significantly in the high dose group.And the the expression of TGF-β₁ and FN in the the model group,the high dose group,the losartan group and the high dose group is gradually descending.Conclusions:Shenqi Jiedu decoction can obviously decease the urin protein, improve the renal function and degrade the adhesion molecule.With regard to the rats of Adenine-induced chronic renal failure,besides increasing ALB,Shenqi Jiedu decoction can elevate the SOD,inhibit the expression of TGF-β₁ and decrease the deposition of FN.And we presume that the mechanisms of Shenqi Jiedu decoction delaying the chronic renal failure correlated with the relief of glomerular sclerosis and interstitial fibrosis.
