Objective:To approach the value of TNF-αin diagnosis of Gastric carcinoma,and to detect gastric carcinoma combined with the carbohydrate-associated antigen(CEA),cancer antigen 125(CA125),carbohydrate antigen 19-9(CA19-9) and carbohydrate-associated antigen 72-4(CA72-4)so as to improve the sensitivity and diagnosis ratio for gastric carcinoma.Methods:The serum concentration of tumor necrosis factor-α(TNF-α), carbohydrate-associated antigen(CEA),cancer antigen 125(CA125),carbohydrate antigen 19-9(CA19-9)and carbohydrate-associated antigen 72-4(CA72-4)in the serum of 96 patients with gastric cancer and 30 healthy individuals were measured by radioimmunoassay(RIA)and quantitative sandwich assay technology Meanwhile,analyzing the difference of a single tumor markers and the combined detection of five tumor markers in sensitivity,specificity and effectiveness for the gastric cancer.Results:In gastric cancer group,the respective content of five tumor markers is TNF-α103.02±182.6 pg/ml,CEA 37.8±25.6ng/mL,CA125 47.9±31.4U/m L and CA19-9 75.9±58.7U/mL,CA72-4 21.7±44.6 U/mL.Compared with the control group,the expression of TNF-α,CEA,CA125,CA19-9,CA72-4 in the gastric cancer group increased significantly.It has statistical significance(P<0.05) and diagnostic value.TNF-αin the diagnosis of stageⅢ,Ⅳgastric cancer has statistically significant(P<0.05).And the later the stages of gastric cancer,the higher TNF-αexpression increased.The respective sensitivity of five tumor markers is TNF-α42.7 percent,CEA 37.5 percent,CA125 21.9 percent,CA19-9 40.6 percent,CA72-4 46.9 percent respectively.In the five tumor markers which we discussed,the sensitivity(46.9 percent)and effectiveness(58.7 percent)of CA72-4 are the highest,followed by the TNF-α,CA19-9,CEA and CA125.So it is the first choice of CA72-4.But it is less sensitive for single tumor marker in the diagnosis of gastric cancer.For single tumor marker,the highest sensitivity could reach to 46.9 percent.Combined detection of five tumor markers could improve the sensitivity rate to 77.1 percent,the specificity rate to 83.3 percent and effective diagnosis rate to 78.6 percent.The results has significant difference(P<0.05), Combined detection of five tumor markers is better than a single test.Conclusion:It has clinical significance in the diagnosis of gastric cancer for tumor necrosis factor-α(TNF-α),carcinoembryonic antigen(CEA),cancer antigen 125(CA125)and carbohydrate-associated antigen CA 19-9,carbohydrate- associated antigen CA72-4.But the positive rate of single indicator in detecting gastric cancer is not high.While the combined detection can improve the diagnosis rate of gastric cancer.So combined detection of tumor markers could improve the diagnosis sensitivity and effectiveness in gastric cancers and was helpful for screening.
