ObjectiveThis study was designed to investigate the constituent ratio of gastroesophageal reflux disease in outpatients of Department of Gastroenterology.Correlation factors thought to be responsible for the disease were studied.MethodsA total of 640 outpatients of Department of Gastroenterology,the First Affiliated Hospital of Medical College,Shantou University aged from 18 to 80 were investigated.Investigation was executed three times monthly from October,2006 to September,2007 randomly.All of the respondents were asked to fill in a questionnaire.The questionnaire was self-designed,including RDQ measuring scale,concomitant symptoms,individual appetite,eating habits and living habits.Diagnostic index was measured by accumulating points of RDQ measuring scale(Sc, highest score 40).Diagnostic criteria of GERD was Sc≥12(including gastroscopy showed RE), others were diagnosed as non-gastroesophageal reflux disease.Results1.Of all the 640 respondents,145 were diagnosed as GERD patients.The constituent ratio of GERD in outpatients of Department of Gastroenterology was 22.7%.2.Of the 145 GERD patients,the frequencies of occurrence of the four symptoms in RDQ, regurgitation(sour or bitter in the mouth),food reflux,substernal chest pain and heartburn,were 84.8%,62.1%,42.8%and 73.1%,respectively.The appearance sequence of concomitant symptoms were eruct(84.8%),epigastric discomfort(73.1%),upper abdominal pain(61.4%), epigastric distention(53.8%),feeling foreign body in pharynx(45.5%),nausea(42.1%), odynophagia(15.2%),laryngopharyngitis(13.1%),chronic cough(11.7%),deglutitio impedita (7.6%) and emesia(3.4%),from upper to lower.3.Dependability between GERD and potential factors3.1 general state Culture degree had nothing to do with GERD,with P>0.05.Age,sex, occupation and obesity were associated with GERD,with P<0.05.When weighed overweight by body mass index(BMI,≥24.0 kg/m~2) and waistline(WC,male≥85cm or female≥80cm),the odds ratios(OR) were 2.052 and 2.744,respectively.And weighed obesity by BMI(≥28.0 kg/m~2) and WC(male>95cm or female>90cm),the OR values were 2.835 and 3.153, respectively.3.2 smoking,drinking tea and alcohol Drinking tea had no significant relationship with GERD(P>0.05).Smoking and drinking alcohol were correlated to GERD(P<0.01),with OR values 2.257 and 1.853,respectively.Further analysis showed that smoking ages and average capacity of alcohol drinking per day were responsible for GERD,while average tobacco quantity smoking per day,drinking ages and category of liquor seemed to have nothing to do with GERD.3.3 eating habits The relation between GERD and not having breakfast usually,eating time irregular,often eating Chaoshan snack,taking sea food commonly,drinking coffee usually and drinking soft drink frequently had no statistical significance(P>0.05).Category of staple food,eating fruits usually,taking vegetable commonly,used to overeating and often taking food at bedtime showed to involve in the occurrence of GERD(P<0.05).Divided category of staple food into sweet food,animal food,plant food,salty food and spicy food,the OR values were 2.468,1.665,0.329,2.769 and 1.780,respectively.The OR values of eating fruits usually,taking vegetable commonly,used to overeating and often taking food at bedtime were 0.541,0.497, 3.483 and 1.949,respectively.3.4 living habits Right arm recumbent,tight clothing or leather belt had nothing to do with GERD(P>0.05).Constipation connected to GERD(P<0.01),with OR value 2.809.Conclusions1.The incidence of GERD is rather high in diseases of digestive system.2.Age,sex,occupation,obesity(including BMI and waistline,waistline more obvious), Smoking,drinking alcohol,category of staple food,eating fruits usually,taking vegetable commonly,used to overeating and often taking food at bedtime are associated with GERD.3.Obesity(including BMI and waistline,waistline more obvious),Smoking(smoking ages chief),drinking alcohol(average capacity of alcohol drinking per day chief),used to overeating, often taking food at bedtime,constipation and taking sweet food,animal food,salty food or spicy food as staple food are GERD's risk factors.Taking plant food as staple food,including eating fruits usually and taking vegetable commonly is a protect factor of GERD.
