Objective:Research on the expression of TNF-αand IL-1βin serum after craniocerebral trauma and the relation with intracranial hypertension after head injury.To determine the application of drugs to lower the intracranial pressure according to the levels of TNF-αand IL-1βin serum.Methods:Based on the prospective study of the 40 craniocerebral trauma cases who were operated on in Department of Neurosurgery of the first affiliated hospital of jinan university from February 2006 to January 2008.According to the Typing of Acute Craniocerebral Trauma amended by China Society of Neurosurgery in 1997,these patients were divided into two groups,20 patients with moderate Craniocerebral Trauma and 20 patients with serious Craniocerebral Trauma.All of them received routine cerebral surgical treatments,Set the transducers of intracranial pressure monitoring below cerebral dura mater in the operating.And the Intracranial Pressure monitored dynamic for 7 days by intracranial pressure monitor.Measured TNF-αand IL-1βin peripheral blood samples from every patient respectively at24h,3dand 7d.Then compared the difference ofintracranial pressure,TNF-αand IL-1βbetween the moderate Craniocerebral Trauma group and the serious Craniocerebral Trauma group using t-test.Compared the Relation of intracranial pressure,TNF-αand IL-1βwith the moderate and the serious Craniocerebral Trauma group by the method of the Spearman's rank correlation.Result:1.Thelevels of intracranial pressure,TNF-αand IL-1βin the serious Craniocerebral Trauma group were higher significantly then the moderate Craniocerebral Trauma group.2.Compared the different time of intracranial pressure,TNF-αand IL-1βbetween the moderate Craniocerebral Trauma group and the serious Craniocerebral Trauma group after operating by the method of the t-test.We can conclude that the intracranial pressure in the serious craniocerebral trauma group was higher than the moderate group(P<0.05),the level of TNF-αand IL-1βin the serious craniocerebral trauma group was higher than the moderate group at24h,3d and 7d(P<0.05) 3.To determine the relation between intracranial pressure and TNF-α,IL-1βin the moderate craniocerebral trauma group and the serious craniocerebral trauma group after operating by the method of Spearman's rank correlation.We could conclude that the positive correlation of intracranial pressure and TNF-αwas confirmed between two groups by the method of the Spearman's rank correlation(r=1.000,1.000,P<0.05);and the positive correlation of intracranial pressure and IL-1βwas confirmed too (r=1.000,1.000,P<0.05)Conclusion.The expression of TNF-αand IL-1βin serum after craniocerebral trauma are obviously higher and they are in positive correlation with intracranial pressure of craniocerebral trauma.So we can determine the application of drugs to lower the pressure according to the levels of TNF-αand IL-1βin serum.
