Objective: To evaluate the prognostic and diagnostic value of heart-type fatty acid binding protein and access the specificity and sensitivity relative to the conventional myocardial biochemical makers.Methods: The whole blood samples were obtained from fifty and two patients who had chest pain at the third, sixth and tweith hour for H-FABP, cTnT,CK-MB,MB and then got the serum. The concentration of H-FABP was measured by a two-step direct sandwich-ELISA method using monoclonal anti-human H-FABP antibodies. And then evaluate the prognostic and diagnostic value for H-FABP, cTnT,CK-MB,MB according to the different ROC curves.Result:1. H-FABP has a higher sensitivity than that of cardiac Troponin (cTnT) ,CK-MB and myoglobin( MB), (0.931) (0.533) (0.567) (0.800) respectively within the third hour after the onset of chest pain(P<0.001). This trend extends to the sixth hour of myocardial injury as well,(0.946) (0.933) (0.900)(0.867);2. The specificity of H-FABP was higher than the cTnT, CK-MB and MB within the sixth hour,( 0.867VS0.929), (0.818VS0.918) (0.818VS0.864), ( 0.827VS0.828), when the time more than six hours, the specificity and sensitivity of cTnT is higher than H-FABP and other biochemical markers. (P<0.05).Conclusion:1. H-FABP was more specificity and sensitivity than conventional myocardial biochemical markers of cTnT, CK-MB,MB at the early hours of myocardial injury particularly within 3 hours after the chest pain.2. The specificity and sensitivity of H-FABP were gradually decreasing while the cTnT and MB were increasing by the time more than 6 hours after the chest pain. So when the time of chest pain for these patients exceeded six hours, combined measurement of H-FABP and these conventional myocardial enzymes may be elevate the diagnostic rate of AMI.
