Objective:To improe and provide guide for clinical aplly of hallucal minu-tissue free flap by further exploring the course,distribution of halllucal blood vessels and its maximeter at different planes,and aplly the flap for repairing tissue defect of the thumb or fingers.Methods:26 fresh foot speciments were dissected,observed and measured,while 40 clinical cases were analyzed.The transplantation of six kinds of hallucal minu-tissue free flaps were applied to repair the tissue defect of the thumb or fingers in 40 cases.Results:The maximal diameter of the hallux fibular artery of base metatarsus was about(0.78±0.05) mm at the proximal 1/3 of distal phalanx,(0.96±0.07) mm at interphalangeal joint position, (1.06±0.16) mm at the distal 1/3 of proximal phalanx,(1.13±0.28) mm at the proximal 1/3 of proximal phalanx,and(1.32±0.31) mm at the position of metatarsophalangeal joint.The maximal diameter of the dorsal hallux veins was about(0.40±0.04) mm at the proximal 1/3 of distal phalanx,(0.52±0.16) mm at interphalangeal joint position,(0.68±0.16) mm at the distal 1/3 of proximal phalanx,(0.81±0.21) mm at the proximal 1/3 of proximal phalanx,and(1.06±0.32) mm at the position of metatarsophalangeal joint.The period of follow up was one to ten months. All flaps were live.The function of the thumb or fingers is good and and their appearance is excellent.Conclusions:The main blood supply on the hallucal free flap is from the haUux fibular artery of base metatarsus,and the other branch artery is miner.Dosal veins of hallux are the most important vessal during tissue flap transplantation,which anastomose with each other widely.The reconstructed thumb or fingers like the original ones with the hallux mini-tissue free flap.The hallucal free flap for repairing tissue defect of the thumb or fingers is a reliable,practical clinical method.
