| As one of the most common women malignant tumors,breast cancer takes 7%~10% of malignant tumors,only less than cervical cancer.But in recent years,the tendency of breast cancer is surpassing that of cervical cancer,and shows an upward trend year after year.Some major cities reported that breast cancer took the first place in women malignant tumors.At present, the level of early diagnosis of breast cancer has been much inproved due to the advances of medical imagelogy and application of invasive methods,which brings the hope of early diagnosis and therapy.In recent years,the therapy of breast cancer showed diversity as a result of the improvement of medical science and thechnology. However, surgical treatment is still a main treatment.non-surgical treatments include:chemotherapy,radiotherapy,endocrine therapy, bio- therapy and traditional Chinese therapy,etc.Comprehensive and individualized treatment of breast cancer research has become a hot spot,which shows the derection to ameliorate prognosis.Surgery based comprehensive treatment is ideal for breast cancer treatment modalities.Breast-conserving therapy is ideal for early breast cancer.Objective Explore the efficacy of comprehensive treatment for breast cancer by comparing the survival rates of patients with surgery alone,with comprehensive treatment and compiling complications of chemotherapy; Explore the efficacy of breast- conserving treatment by comparing the relapse rate,distant metastasis rate,survival rate and incidence of postoperation complications of breast-conserving group and modified radical mastectomy group.Methods Retrospectively analyse the 2-year-median follow-up surgical treatment of 186 breast cancer cases of clinical phase I ~ III. According to whether given other treatments,These cases were divided into simple operation group(21 cases) and comprehensive treatment group(165 cases).Compare the survival rate of the two groups and compile statistics of complications in 156 chemotherapy patients;119 Phase I&II breast cancer patients who took comprehensive treatment,were divided into breast -conserving group(32 cases) and modified radical mastectomy group(87 cases).Compare the recurernce rate,distant metastasis rate,survival rate and incidence of postoperative complications of the two groups.Results The survival rate of operation is 85.7%,while the comprehensive treatment group is 99.4%, which have statistically significant differences (P<0.05).Recurrence rate,distant metastasis rate,survival rate,incision infection rate,upper limb edema rate of breast-conserving group are 0%, 0%,100%,3.1%,0%,while those 0%, 1.1%,98.9%,2.3%,1.1% in modified radical mastectomy group.No significant differences were found between the two groups(P>0.05). Rates of subcutaneous fluid,skin flap necrosis are 3.1% and 0% in breast-conserving group, while 19.5% and 11.5% in modified radical mastectomy group,which have statistically significant differences (P<0.05).The incidence of chemotherapy complications statistics: necrosis 0%, phlebitis 0%, abnormal liver function 22.8%, nausea and vomiting 78.8%,drop in white blood cell count 22.8%, allergic reaction 1.9%, infection 1.3%, limb numbness 0.6%, cardiac toxicity 0%.Conclusion Comprehensive treatment of operation,together with chemical, radiological, endocrine, immune therapy, is an ideal mode for breast cancer and can be used to improve survival rate; Breast-conserving therapy shows similar efficacy to modified radical mastectomy in Phase I&II breast cancer,and has less trauma,less complications,better cosmetic effect and higher quality of life.It is ideal for the treatment of early breast cancer;Active prevention and treatment for complications of chemotherapy can improve the efficacy of treatment for breast cancer. |
