| OBJECTIVE To establish a new sensitive method for the simultaneous determination of L-Carnitine(LC),acetyl-L-Carnitine(ALC)and propionyl-L-Carnitine (PLC) in Human Plasma by HPLC.And to investigate the relations between the transplanted kidney and LC,ALC or PLC in human plasma.METHODS The analytes were extracted by protein precipitation and then precolumn derivatization with 1-aminoanthracene(I-AA) were performed.The fluorescent derivatives were separated on a Hypersil C18 column,and the mobile phase consisted of acetonitrile-0.1 mol·L-1 ammonium acetate(28:72),the flow rate was 1.0ml·min(-1).The derivatives were monitored with a fluorimetric detector set at 248 nm(excitation wavelength) and 418 nm(emission wavelength).The results of clinic plasma sample are tested by SPSS11.5,to analyze the significant difference of LC,ALC and PLC between Pre- and post-operative,and the dependability with creatinine clearance rate(Ccr).To estimate the relativity between the transplanted kidney function and LC,ALC or PLC.RESULTS The good linearity of the assay was observed over the concentration ranges investigated,5-160μmol·L-1 for LC,1-32μmol·L-1 for ALC,0.25-8μmol·L-1 for PLC.The recoveries of LC,ALC and PLC were 99.48%,99.94%and 98.20%, respectively.The intra- and inter-day validation were adequate with the RSDs of 5.83%or below.LC and ALC are significant difference between pro- and post- Kidney transplantation(P＜0.05),but PLC is not.At the time,the 10day and the 30day after Kidney transplantation,the coefficient correlation of LC,ALC,PLC with Ccr is -0.861, -0.834,-0.190 and -0.708,-0.715,-0.176.CONCLUSION This method is proved to be stable,accurate and convenient to study the concentrations of LC,ALC and PLC in clinical plasma.LC,ALC in plasma are affinitive with the function of the transplanted kidney,PLC is opposite.
