Objective: To detect the immunological reaction and cell proliferation in the breast tissues of breast augmentation patients with medical hydrophilic polyacrylamide gel injection, and observe the changes of ultrastructure in the galactophore by Transmission Electron Microscope.Methods:1. We detected the expressions of CD68,CD25,PCNA,HSP70,P53 and C3 in 20 cases with nodules, 12 cases without nodules of 32 cases with breast augmentation by polyacrylamide hydrogel injection, and 10 normal breast tissues by immuno- histochemistry P-V6000, and analyzed all the samples combining with HE staining.2. Tissues of 1 mm×1mm×1 mm in volume in the same places were picked up and made into extra-thin slices, which ultrastructural change were observed by Transmission Electron Microscope.Results:1. Fibrous tissues proliferation can be seen in the tissues besides the injected PAAHG, and plenty of mechanocytes and histiocytes but lymphocytes accumulates surrounded. Large number of multinucleate giant cells and inflammatory cells can also be seen around the PAAHG, which showed blue-purple by HE stain and usually induced granuloma fomation witn thin fibrous stratum .2. Positive cells of CD68, PCNA, HSP70,P53and C3 were hardly founded in normal tissues, while lots of them were observed in the breast tissues around polyacrylamide hydrogel, and especially obvious in 12 cases with nodules. The differences were significant stastically. With respect to CD25, there are no significant differences among the three groups. Positive cells of P53 were not seen in the tissue around hydrogel and there are also no significant differences. The expression of PCNA and CD68 form direct ratio with implantation time.3. Transmission Electron Microscope showed that endoplasmic reticulum of fibroblast cells swelled, rough endoplasmic reticulum seldom had degranulation and part of cristaes disruptd, part of nuclear membrane discontinue and nucleus vacuolization could be seen hardly. Little Ribosome and copious collagen fiber and gel tissue can be found in the majority of fibroblast cells.Conclusions:1. From our experiment we found that the implantation of PAAHG into the women breast tissues can cause local immunological reaction and cell proliferation. Furthermore, the reaction of nodule group are much more severe than non-nodule group , by different degrees which is correlated with the implantation time .which indicates that PAAHG should be taken out as soon as possible for mammoplasty patients with mammary nodules in order to release the local damage.2. The implantation of PAAHG into the tissues doesn't initiated mutations of P53.3. PAAHG injection can lead to ultrastructurral changes of the mechanocytes , which indicates that implantation of PAAHG can induce the degeneration of fibroblast cells in the women breast tissues.On the whole, our study on the cases with 3-8 years history showed that PAAHG were harmful to the surrounded mammary tissures. More cases and longer follow-up investigation are needed as to further conclusions.
