| Background: Viral myocarditis is one of the myocarditis in which heart is infected by virus. We still know little about the mechanism, therapeusis and prognosis of viral myocarditis, even though it has been studied for decades throughout the world.More and more studies show that viral myocarditis may result in heart failure, arrhythmia and dilated cardiomyopathy. More and more people believe that autoantibodies, including anti-cTnI etc,play an important role in the process of viral myocarditis and may result in a bad prognose. So it is necessary for us to study the prognosis of viral myocarditis and the clinical value of anti- cTnI in predicting the prognosis of viral myocarditis.Objectives: To visit the patients of viral myocarditis who were once inpatients in Zhongshan hospital and work out a rough constituent ratio about the prognosis of viral myocarditis.To study the positive rate of anti-cTnI in people of health, viral myocarditis and dilated cardiomyopathy in order to show its relationship with the diseases and its clinical value in predicting the prognosis of viral myocarditis.Methods: To enrol datas of inpatients of viral myocarditis in Zhongshan hospital since 2003 to 2005. To follow up all the patients by letters,phones and visitations in order to make a rough conception of the prognosis of viral myocarditis.To compare the proportion of prognosis of mild viral myocarditis with severe ones.To test anti-cTnI in the bloods of people of health,mild and severe viral myocarditis and dilated cardiomyopathy in order to compare the positive rate between these groups,in order to show its relationship with the degree of diseases and in order to estimate the clinical value of anti-cTnI in predicting the prognosis of viral myocarditis.Results: About twenty-three percent of viral myocarditis are severe.There are no differences in prognosis of recovery, arrhythmia and therapeutics of pacemaker between mild and severe viral myocarditis,but there is an obvious difference between two diseases in mortality rate(P=0.002).The risk of dilated cardiomyopathy caused by severe viral myocarditis is sixteen times more than that caused by mild viral myocarditis. Anti-cTnI can be observed in the blood of people not only of viral myocarditis and dilated cardiomyopathy,but also of health.The titer of anti-cTnI of each group is different.The positive rates of anti-cTnI in mild viral myocarditis, severe viral myocarditis and dilated cardiomyopathy are 25%,60% and 65%.Conclusions: The morbidity of severe viral myocarditis in viral myocarditis is among 14.95% to 32.61%.There is a different mortality in statistics between severe viral myocarditis and mild viral myocarditis.But there is no difference in a long term survival rate.The risk of dilated cardiomyopathy caused by severe viral myocarditis is sixteen times more than that caused by mild viral myocarditis. Anti-cTnI can be observed in the blood of people not only of viral myocarditis and dilated cardiomyopathy,but also of health. The titer of anti-cTnI is different in health people from severe viral myocarditis or dilated cardiomyopathy. The titer of anti-cTnI has a relationship with degree of disease. The cTnI is not the only antigen of anti-cTnI in human beings. The positive rates of anti-cTnI in mild viral myocarditis, severe viral myocarditis and dilated cardiomyopathy are 25%,60% and 65%.There is a difference in statistics of the positive rates of anti-cTnI between health people and severe viral myocarditis or dilated cardiomyopathy. It is show that anti-cTnI stands for the damage of myocardium and it may help to provide a new test mark in predicting the prognosis of viral myocarditis. |
