BackgroundHelicobacter pylori infection is a significant pathogenic mechanism and independent dangerous factor for precancerous lesions such as peptic ulcer or gastric carcinoma,thus,the working group of International Agency for Research on Cancer (IARC) of World Health Organization have already classified it as a class I carcinogen.The Helicobacter pylori IgG in human saliva leaks out through the gingival pocket from the Helicobacter pylori IgG which exist in blood that can be detected by hypersensitive Method.Many research at home and abroad have shown that the tite of Helicobacter pylori IgG in saliva and blood was positively correlated and the Helicobacter pylori IgG in saliva can be an indicator of HP infection.The current researchs have shown that the content of Helicobacter pylori IgG in Helicobacter pylori positive gastric juice and gastric mucosa is closely related to tissular change. After Helicobacter pylori infected, mucosal atrophy, dysplasia and cancer are all closely related to the Humoral immune reaction of gastrointestinal tract to Helicobacter pylori . Histologic characteristics of gastric carcinoma and precancerous lesions are possibily the response of gastric mucosa to the persistent existence of Helicobacter pylori antigen.ObjectiveTo analyze the statistical difference of the content of Helicobacter pylori IgG in human saliva and gastric juice between precancerous lesions and gastric carcinoma and also explore the initial value of detecting Helicobacter pylori IgG in human saliva to screen gastric cancer. MethodsDetecting the content of Helicobacter pylori IgG in human saliva and gastric juice of chronic atrophic gastritis group, gastric ulcer group and gastric cancer group and control group. The content of Helicobacter pylori IgG is demonstrated by the optical density value at 450nm in microplate reader.ResultsThe optical density value of Helicobacter pylori IgG in human saliva and gastric juice in control group are 0.156±0.102 and 0.237±0.125. The optical density value of Helicobacter pylori IgG in human saliva and gastric juice in chronic atrophic gastritis group are 0.431±0.131 and 0.752±0.157. The optical density value of Helicobacter pylori IgG in human saliva and gastric juice in gastric ulcer group are 0.441±0.082 and 0.824±0.116. The optical density value of Helicobacter pylori IgG in human saliva and gastric juice in gastric cancer group are 0.581±0.150 and 1.132±0.209. Compared with the control group,the difference of the optical density value of Helicobacter pylori IgG is of statistical significance in human saliva and gastric juice between chronic atrophic gastritis group, gastric ulcer group and gastric cancer group(P <0.05). It is of obviously statistical significance that the optical density value of Helicobacter pylori IgG in human saliva and gastric juice in gastric cancer group is higher than it in the chronic atrophic gastritis group or gastric ulcer group (P <0.01). It is of no statistical significance that the optical density value of Helicobacter pylori IgG between the chronic atrophic gastritis group and the gastric ulcer group (P >0.05). It is of the most effectivity when taking the optical density value of Helicobacter pylori IgG greater than or equal to 0.50 as the standard to initially screen gastric cancer. And it is also consistent with the description of the Kit to judge the positive criteria. The sensitivity of the standard is 72.0%,the false positive rate of it is 35.71% and the false negative rate of it is 28.0%. ConclusionsThe difference of the content of Helicobacter pylori IgG in human saliva and gastric juice is of statistical significance between precancerous lesions and gastric carcinoma. It is of obviously statistical significance that the content of Helicobacter pylori IgG in patients with gastric cancer is higher than that in patients with chronic atrophic gastritis or gastric ulcer. It is an effective means to take the optical density value of Helicobacter pylori IgG greater than or equal to 0.50 as the standard to initially screen gastric cancer.
