Clinical Research Of Blood Hypercoagulabale State Of Patient Who Plan Making Radical Resection Of Rectal Carcinoma In Period Of Operation And Ulinastatin Interfered

Objective:Primary part:to investigate platelet mobilizing function,blood clotting function and platelet correlated parametric detection index of rectal cancer patient and healthy volunteer,to dentify correlation of malignant tumor sufferer and blood hypercoagulabale state;Second part:to investigate influence to platelet mobilizing function,blood clotting function and platelet correlated parametric detection index of rectal cancer patient Ulinastatin interfered,to provide theory to utility measure which ameliorate blood hypercoagulabale state of rectal cancer patient.Methods:Primary part:platelet mobilizing function,blood clotting function and platelet correlated parametric detection index of rectal cancer patient who have gotten patho- final diagnosis are measured,Forty healthy people served as the control. Interclass of general state of health is not difference.Second part.Thrity ASAⅠorⅡpatients without blood dyscrasia and blood coagulation obstacle(age:45～65y;weight: 50～70kg;man:17,women:13) of rectal adenocarcinoma(TNM,Ⅱ-Ⅲstage) and scheduled for Dixon operation were randomly allocated to two groups(n= 15,each): GroupⅠ(UTI group,n =15 ) in which patients received UTI 5000 U/kg,and GroupⅡ(control group,n = 15 )in which patients received NS 100 ml.The use of narcotic and anaesthesia were identical in operation.Activation markers of platelet(CD62p,CD63),blood clotting function correlated parameter(PT,APTT,TT,FIB) and platelet correlated parameter(PLT,PDW,MPV,P-LCR)were measured at 3 points: pre-infusion(TO),1 hour after the infusion(T1),and 2 hours after the infusion (T2).Results:Primary part:There were no significant difference in age,body height,body weight,ASA Class,TNM class in the two groups(P＞0.05).Compared with health adults',Activation markers of platelet CD62p,CD63,platelet correlated parameter PLT,PDW,MPV,P-LCR and blood clotting function correlated parameter FIB were obviously increased,(P＜0.05 ),especially CD62p,CD63(P = 0.000),but there were no significant difference in the blood clotting function correlated parameter PT,APTT,TT;Second part:There were no significant difference in age,body height, body weight,ASA Class,TNM class in the two groups(P＞0.05).Compared with groupⅡ(controlled group):Activation markers of platelet CD62p,CD63 in groupⅠwere obviously decreased at T1 and T2.APTT were prolonged significantly at T1(P＜0.01 ).TT were prolonged significantly at T2,(P＜0.05).PT were prolonged significantly at T1 and T2,(P＜0.05 ),MPV,PDW,P-LCR were obviously decreased at T1 and T2,(P＜0.05).Compared with T0(pre-infusion),ativation markers of platelet CD62p,CD63 were obviously decreased at T1 and T2,(P＜0.05),APTT were prolonged significantly at T1,(P＜0.01).TT were prolonged significantly at T2,(P＜0.05).PT were prolonged significantly at T1 and T2,(P＜0.05),there were no significant difference in PLT,MPV,PDW,P-LCR at T1 and T2.Conclusion:(1) Compared with health adults',Activation markers of platelet CD62p,CD63,platelet correlated parameter PLT,PDW,MPV,P-LCR and blood clotting function correlated parameter FIB were obviously increased,indicate blood hypercoagulabale state usually reside in rectal cancer patient,who is the high-risk group of venous thrombosis.(2)UTI5000U/kg can ameliorate rectal blood coagulation status of cancer patients,which may reduce microthrombus syndrome in the operation and prevent venous thrombosis after the operation...
