Objective:To explore and compare with the effects of early individualised parenteral nutrition for premature infants.Methods:Total of 56 survival premature infants with low birth weight or very low birth weight in our hospital were divided into two groups randomly.Twenty five were in experimental group,mean birth weight was1645±265g,gestational age was33.21±2.12w. The infants were treated in the early individualised parenteral nutrition.Amino acid solution and fat emulsion were started beginning on the first day of life.Amino acids were 1.0g/kg.d,increased by 0.5g/kg.d up to a maximum of 3.5g/kg.d.Lipids were 0.5g/kg/d, increased by 0.5g/kg.d up to 3.0g/kg.d.Thirty one were in control group,mean birth weight was1698±200g,gestational age was33.05±1.80w.The infants were tread in the traditional parenteral nutrition in our neonatal department.The average given total calorie per day in the first week,the average time of 50%needed calorie in enteral nutrition,the average time of weight regain to birth weight,the average weight gain per day when recovery birth weight,the average of head circumference,arm circumference and body length per week were calculated.Blood samples for total bilirubin,indirect bilinrubin, glutamate-pyruvate,urea nitrogen,total cholesterol,triglyeride,total protein,albumin, globulin,prealbumin in all infants on the first,7th,14th day after birth were compared in two groups.Statistical analysis was done using Student's t test or X2 test.Result:1.Compared with the control group,experimental group was given more total calorie in the first week(P<0.01).There was no difference in the average time of 50% needed calorie in enteral nutrition between two groups(P>0.05).2.Compared with the control group,experimental group had a shorter time of regain birth weight,The deference was highly significant(P<0.01),more rapid weight gain after recover birth weight,The deference was also highly significant(P<0.01).3.Compared with the control group, experimental group had a good head circumference(P<0.05).There were no differences in arm circumference and body length between two groups(P>0.05).4.There were no differences of the concentrations of total bilirubin,glutamate-pyruvate,urea nitrogen,total protein,albumin,globulin between two groups.Compared with the control group, experimental group had a lower concentration of indirect bilinrubin(P<0.05) and a higher total cholesterol(P<0.01)in the first week of parenteral nutrition,and had a higher prealbumin(p<0.05) in the second week of parenteral nutrition.5.The incidence of hyperglycemia was lower in experimental group than control.Conclusion:The early individualised parenteral nutrition could raise the energy supply,shortting the time of weigt regain,increase the weight gain and head circumference, decrease complications of preterm infants and without apparent side effects.
