| Objective:To discuss selection technique of artificial cotyle and curative effect of cotyle coloboma treatment in total hip arthroplasty.Methods:In the investigation for 26 cases between August of 2004 to October of 2007,10 cases for male,12 cases for female and 4 cases had not followed up,their average age 63.3, Left replacement cases 18 and right replacement cases 4.whys:15 cases of osteolysis with artificial cotyle looseness,3 cases of infection,1 case of cotyle malposition,1 case of cotyle abrasion,1 case of femoral shaft fracture,1 case of Femoral shaft reveal.According to the classification of AAOS for Cotyle abrasion,â… type 5 cases,â…¡type 4 cases,â…¢type 13 cases.In which,for ten cases,we adopted bone grafting of pressed-granule-type artificial biomaterial or fortified cement-type cotyle.We evaluated the treatment and curative effect after operation with Harris point Standard and X-ray.Result:In the follow-up visit to 22 cases patients from 26 cases,visit time 15-53 months, average visit time 30.8 months,average Harris point was raised from(38.5±8.8)(21-50) before the operation to(82.3±6.3)(66-89)(P<0.05)after the operation.Hip joint function of 22 cases patients was improved and pain was disappeared or disappeared basically,sinus of infected persons was healed up without any pus.Good curative effect 15 cases,basically good 6 cases and failed case 1.from X-ray film,we didn't found any complication such as bone graft absorption,infectious diseases,rejection reaction and artificial cotyle looseness.We can partly found continious bone trabecula between grafting bone and governing bones.Conclusion:In operation of Hip joint Replacement,basic guarantee of successful operation is that we should prepare all kinds of artificial cortile and select proper one for operation of autotransplantation and alloraft according to different types of cotyle coloboma, and Guiding exercises and follow-up visit is also very important.The recent follow-up visit show that this kink of hipbone replacement operation is of good Curative Effect and clinical application. |
