Diagnosis And Treatment Of Solid Pseudopapillary Tumor Of The Pancreas

Objective To analyze the origin,clinical and pathological characteristics,diagnosis, treatment and prognosis of solid pseudopapillary tumor of the pancreas(SPT).Method 11 cases with SPT in our hospital were analyzed with the method of immunohistochemistry. 25 cases in our hospital and 495 cases reported in Chinese literature from 1993.1 to 2009.2 were analyzed.Date about the clinical pathological characteristics,diagnosis,differential diagnosis,treatments and prognosis was collected.Result The average age of the patients was 24.09 years.The percent of female patients was 90.8%(465/512cases).91 patients(19.12%) were asymptomatic except for the palpable abdominal Mass.181 patients(38.03%) complained of vague abdominal pain.176 patients(36.76%) were found neoplasms by medical examination.The tumors were 6.85 cm in average diameter,and were located in the head(178 patients,35.18%),neck(49 patients,9.68%) and body or tail(262 patients,51.79%).On the CT scan,all the SPTs demonstrated solid and cystic mixed structures with low density.And the tumor amicula was enhanced in the arterial phase,especially in the venous phase.The SPTs With hypersplenotrophy and thicker of tumor amicula may be malignant.97.8%cases were resected radically.295 patients were followed up with the period from 1 to 108 months(average 32.5 mon).And only 9 patients were reported died.Conclusion 1 SPT is a special type pancreatic tumor with low degree malignancy that almost all occurs in the female.The disease may origin from multipotential stem cell and was correlated with sexual hormone.CT(computed tomography) is helpful to detect whether it is malignant.Surgery is the main treatment method.
